06-11 Alcohol Diarrhea 酒泄 주설
① When one drinks alcohol too much, it eventually becomes alcohol diarrhea. The person becomes skinny and will not be able to take in food, drinking only one or two glasses of water. If it is not cured for several years, a Frankincense and Deer Antler Velvet Pill (香茸丸) should be prescribed.
② For the person suffering from alcohol diarrhea, when it is especially severe after drinking alcohol, 2 don of Calm Stomach Powder (平胃散) with the addition of Syzygii Flos (丁香), Amomi Fructus (砂仁), Puerariae Radix (葛根), Hordei Vulgaris Fructus (麥芽), and Massa Medicata Fermentata (神麯), powdered and taken with thin rice gruel, should be prescribed on an empty stomach.
③ When the patient has diarrhea at early dawn from alcohol damage, 2 don of a Wine Steamed Coptis Pill together with an Order the Center Decoction, with the addition of Zingiberis Rhizoma (生薑) and Puerariae Radix (葛根), should be taken on an empty stomach. It has a great effect.
- Frankincense and Deer Antler Velvet Pill 香茸丸 향용환: It treats alcohol diarrhea.
Olibanum (乳香) 3 don, Cervi Parvum Cornu (鹿茸) (remove fur by burning in fire and roasting after being covered with condensed milk) 5 don, Myristicae Semen (肉豆蔲) (cut on both sides, Olibanum placed in the gaps, coated with flour, and baked under an ash fire) 1 nyang, and Moschus (麝香) 2 don (ground separately). Powder these medicines, make into a paste with steamed rice made of old rice, and form pills the size of a foxglove seed. Take 50 pills with porridge.
06-12 Spleen Diarrhea 脾泄 비설
① The patient with spleen diarrhea has heavy limbs, bodily discomfort, stomach discomfort, yellowish complexion, and abdominal bloating. Herbs such as Atractylodis Rhizoma (蒼朮), Magnoliae Cortex (厚朴), Aucklandiae Radix (木香), Zingiberis Rhizoma (乾薑), and raw Myristicae Semen (肉豆蔲) should be considered.
② In spleen diarrhea, the patient feels bloated after meals and comfortable once diarrhea occurs, with a fine pulse. An Aucklandia and Amomum Six Gentlemen Decoction should be prescribed.
- Aucklandia and Amomum Six Gentlemen Decoction 香砂六君子湯 향사육군자탕: It treats spleen diarrhea. Cyperi Rhizoma (香附子), Amomi Fructus (砂仁), Magnoliae Cortex (厚朴), Citrus Unshius Pericarpium (陳皮), Ginseng Radix (人參), Atractylodis Rhizoma (蒼朮), Paeoniae Radix (芍藥) (fried), Atractylodis Rhizoma (蒼朮) (fried), Dioscoreae Rhizoma (山藥) (fried) 1 don each, and Glycyrrhizae Radix (甘草) (baked) 5 pun. Slice as one pack, add 3 pieces of Zingiberis Rhizoma (乾薑) and 1 Mume Fructus Praeparatum (烏梅), decoct in water, and take.
- Evodia Decoction 吳茱萸湯 오수유탕: It treats spleen diarrhea. Boil Evodiae Fructus (吳茱萸) 5 don in plain water and remove the residue. Add a little salt and take. Evodia warms the bladder, allowing urine to flow smoothly and strengthening the large intestine. Although other medicines can warm, they cannot separate urine and feces.
07 For Diarrhea, Upraising Medicine Should Be Prescribed 泄瀉宜用升陽之藥 설사에는 승양시키는 약을 써야 한다
Many people suffer from diarrhea in the rainy season of summer. It is because there are many cases in which dampness induces the five diarrhea. In the Inner Classic (內經), it is said, “If there is a pathogen in the lower energizer, it should be removed through inducing diarrhea.” and “When urination is still difficult in the treatment of dampness, then the treatment is not enough.”
In treatment, the difficulty of urine should be relieved with light medicines. However, if the pathogen of cold-dampness invades inside, the disease will be aggravated. When urine-easing medicine is used in this case, descending qi moves even more down, and yin is increased so yang gets more exhausted. Therefore, medicines that make yang raised should be prescribed.
Osterici Radix (羌活), Araliae Continentalis Radix (獨活), Cimicifugae Rhizoma (升麻) 1.5 don each, Saposhnikoviae Radix (防風), and Glycyrrhizae Radix (甘草) (baked) 1 don each. Slice these medicines, decoct with water, and take.
In the Great Rites (大法) it is said, “When cold-dampness is in excess, it should be harmonized by helping wind.” It is also said, “Raise up things drooping down,” and this is the case.
08 Differences Between Diarrhea and Dysentery 瀉與痢不同 설사와 이질의 차이점
Symptoms of diarrhea are feces coming out no matter whether food and drink is ingested or not and tiredness. Symptoms of dysentery are different. There may be pus, blood, oil stains, or residue in dysentery.
Although pain exists in diarrhea and no pain exists in dysentery, abdominal pain, and heaviness of the bottom exist in both cases. Also, red feces comes out in diarrhea and white feces in dysentery.
09 Chronic Diarrhea Becomes Dysentery 久泄成痢 구설이 이질이 된다
① When the greater yin meridian receives dampness, watery diarrhea continues, the body feels heavy, the abdomen is bloated, and appetite is lost. When this is prolonged, it changes into pus-blood dysentery.
② Dysentery results from the disharmony of nutrient-defense and weakened intestines. Cold qi or heat qi invades between the deficiency of the intestines to cause dysentery or diarrhea.
③ Loss of moderation in food and living damages stomach qi. This pulls ascending clear qi down, and so intaken food is released in diarrhea. If this is prolonged and moves from greater yin to lesser yin, it becomes dysentery.
10 Many Kinds of Dysentery 痢疾諸證 여러 가지 이질
① In congested diarrhea, the Inner Classic (內經) introduces blood overflowing, blood diarrhea, blood feces, and water diarrhea. Other classics introduce passing stools with blood and pus and downward diarrhea. As these are called dysentery in modern days, they are actually the same thing.
② There are red dysentery, white dysentery, red-white dysentery, water-food dysentery, pus-blood dysentery, food-denying dysentery, intermittent dysentery, wind dysentery, cold dysentery, dampness dysentery, heat dysentery, qi dysentery, deficient dysentery, continuous dysentery, accumulation dysentery, chronic dysentery, epidemic dysentery, parasitic dysentery, and five colors dysentery.
10-1 Red Dysentery 赤痢 적리
① Red dysentery is caused by dampness-heat from the small intestine.
② Red dysentery is blood dysentery in other words.
③ When a patient has diarrhea that contains red masses and has fever over the body, a Preserve Harmony Pill with water boiled with Augment Base Powder with the addition of Tetrapanacis Medulla (木通), Paeoniae Radix (芍藥) (fried), Citrus Unshius Pericarpium (陳皮), and Atractylodis Rhizoma (蒼朮) should be prescribed.
④ A Guide Out Red Formula with Sanguisorba, Modified Calm Stomach Powder, Sanguisorba Powder, Stabilize Yang Pill, or Clear Six Pill (淸六丸) should be prescribed for red dysentery.
- Guide Out Red Sanguisorba Decoction 導赤地楡湯 도적지유탕: It treats red dysentery and blood dysentery. Sanguisorbae Radix (地楡), Angelica Gigantis Radix (當歸) (body part washed with alcohol) 1.5 don each, Paeoniae Radix (赤芍藥) (fried), Coptidis Rhizoma (黃連) (fried after wetted with alcohol), Scutellariae Radix (黃芩) (fried after wetted with alcohol), Sophorae Flos (槐花) (fried) 1 don each, Asini Corii Colla (阿膠珠), Schizonepetae Spica (荊芥穗) 8 pun each, and Glycyrrhizae Radix (甘草) (baked) 5 pun. Slice these medicines as 1 pack, decoct with water, and take on an empty stomach.
- Modified Calm Stomach Powder 加減平胃散 가감평위산: It treats blood dysentery and red dysentery. If the spleen and stomach are deficient, blood does not flow to the limbs but rather enters into the stomach, and becomes blood dysentery. Atractylodis Rhizoma (蒼朮), Magnoliae Cortex (厚朴), Citrus Unshius Pericarpium (陳皮) 1.2 don each, Persicae Semen (桃仁), Ginseng Radix (人參), Coptidis Rhizoma (黃連), Asini Corii Colla (阿膠珠), Poria Sclerotium (茯苓) 7 pun each, Glycyrrhizae Radix (甘草) 9 pun, Aucklandiae Radix (木香), and Arecae Semen (檳榔) 5 pun each. Slice these medicines as 1 pack, add 3 pieces of Zingiberis Rhizoma (乾薑), and 2 Zizyphi Fructus (大棗), decoct with water, and take on an empty stomach.
- Sanguisorba Powder 地楡散 지유산: It treats blood dysentery and red dysentery. Sanguisorbae Radix (地楡), Paeoniae Radix (赤芍藥), Coptidis Rhizoma (黃連), and Citri Unshius Pericarpium Immaturus (靑皮) all the same quantities. Powder these medicines, mix 3 don each in watery porridge, and take on an empty stomach.
- Stabilize Yang Pill 固腸丸 고장환: ① It treats blood dysentery and red dysentery. After the accumulation in the intestines is removed, the dampness of the lower energizer should be removed with this medicine. Dry Ailanthi Radicis Cortex (樗根白皮) over a fire and grind the paste into gruel, and then make into pills the size of a seed of a foxglove tree. Take 20~30 pills at a time with gruel on an empty stomach. Since this medicine is cool and dry in property, it must be fried before use.
② Grind Ailanthi Radicis Cortex (樗根白皮) 4 nyang and Talcum 2 nyang and make into pills with gruel.
10-2 White Dysentery 白痢 백리
① As white dysentery belongs to qi, it is caused by dampness-heat from the large intestine.
② Because dysentery comes from accumulated dampness-heat in the intestines and stomach, they are all symptoms of heat. Although dysentery is considered a cold symptom, it is wrong. For example, when white pus comes out due to heat caused by the abscess, can it be considered as a cold symptom because it is white? If the white color is a cold symptom and red color is a heat symptom, is it a disease of cold-heat severely accumulated in the intestines and stomach if the white and red colors are mixed? Moreover, it is surely a heat symptom because there are many cases that stools come down pressing so the bottom is uncomfortable and painful, and white dysentery is shown with red and difficult urine which is a sign that it has been attacked by heat.
③ When diarrhea or dysentery is not treated with warming the spleen, then the kidneys should be warmed. It is because the kidneys control bone marrow, and white mucous stool is another name for bone marrow. The symptoms are that the patient has a dark complexion and the bones are weak. It is surely kidney-deficiency so you have to use medicines like Psoraleae Semen (破故紙), Angelica Gigantis Radix (當歸), Aucklandiae Radix (木香), Zingiberis Rhizoma (乾薑), and Cinnamomi Cortex (桂皮).
④ Augment Base Powder Warm Six Pill, Stewed Aucklandia Pill, or Drain White to Calm Stomach Drink should be prescribed for white dysentery.
- Warm Six Pill 溫六丸 온육환: It treats white dysentery. This is one je of Augment Base Powder (益元散) added with 1 nyang of Zingiberis Rhizoma (乾薑). Powder these medicines, make into dough with steamed rice, and make into pills the size of a seed of a foxglove tree. Take 50~70 pills with boiled water.
- Stewed Aucklandia Pill 水煮木香元 수자목향원: It treats white dysentery and light-red dysentery. Zingiberis Rhizoma (乾薑) 2 nyang, Pericarpium Papaveris (罌粟殼) 1 nyang, Terminaliae Fructus (訶子肉) 3 don, Angelica Gigantis Radix (當歸), Paeoniae Radix (芍藥) 2.5 don each, Aucklandiae Radix (木香), Citri Unshius Pericarpium Immaturus (靑皮), Citrus Unshius Pericarpium (陳皮), and Glycyrrhizae Radix (甘草) 1.5 don each. Powder these medicines, make into dough with honey, and make into pills to the size of a bullet. Melt one pill with boiled water and take.
- Drain White to Calm Stomach Drink 瀉白安胃飮 사백안위음: It treats white dysentery. Atractylodis Rhizoma (蒼朮) (fried), Paeoniae Radix (芍藥) (fried after wetted with alcohol), Nelumbinis Semen (蓮肉) 1 don each, Atractylodis Rhizoma (蒼朮) 7.5 pun, Ginseng Radix (人參), Citrus Unshius Pericarpium (陳皮), Poria Sclerotium (白茯苓), Astragali Radix (黃芪) (fried after wetted with honey), Angelica Gigantis Radix (當歸) (washed with alcohol) 5 pun each, Aucklandiae Radix (木香), Zingiberis Rhizoma (乾薑) (baked after wrapped with watery paper), and Glycyrrhizae Radix (甘草) (baked) 3 pun each. Slice these medicines as 1 pack, decoct with water, and take on an empty stomach.
10-3 Red-white Dysentery 赤白痢 적백리
① When cold qi and heat qi are uneven, and the red and white colors are both shown in the feces, a Zingiberis Ink Pill should be prescribed. When the patient has intermittent diarrhea that resembles dysentery, a Coptis and Evodia Pill should be prescribed.
② When feces are red or white as cold qi and heat qi are uneven, a Minor Stabilizing Formula should be prescribed.
③ A Coptis and Ass Hide Gelatin Formula, Stabilize Yang Decoction, Person of Integrity Maintain Organs Decoction, or Coptis and Evodia Pill should be prescribed for red-white dysentery.
- Zingiberis Ink Pill 薑墨丸 강묵환: It treats red-white dysentery and parasitic dysentery.
Zingiberis Rhizoma (乾薑) (fried) and Pini Atramentum (松烟墨) (heated). Powder the same amounts of these two medicines, make into paste by combining with vinegar, and make into pills the size of a seed of a foxglove tree. Take 30~50 pills with porridge on an empty stomach three times a day. - Coptis and Evodia Pill 茱連丸 수련환: ① It treats red-white dysentery. Evodiae Fructus (吳茱萸) and Coptidis Rhizoma (黃連) 2 nyang each. Soak these medicines in a good alcohol for 3 days. Take each of them out separately, dry over a fire and powder, make a paste by combining with vinegar, and make into pills the size of a seed of a foxglove tree. Swallow 30 pills with water decocted with Glycyrrhizae Radix (甘草) for red dysentery, swallow 30 pills of Evodiae Pills with water decocted with Zingiberis Rhizoma (乾薑) for white dysentery, take 30 pills each with water decocted with Glycyrrhizae Radix (甘草) and Zingiberis Rhizoma (乾薑) for red-white dysentery.
② This is also called a Coptis Pill (黃連丸).
- Minor Stabilizing Formula 小駐車元 소주거원: It treats red-white dysentery. Coptidis Rhizoma (黃連) 3 nyang, Asini Corii Colla (阿膠珠) 1.5 nyang, Angelica Gigantis Radix (當歸) 1 nyang, and Zingiberis Rhizoma (乾薑) 5 don. Powder these medicines, make into paste by adding vinegar and make into pills the size of a seed of a foxglove tree. Take 30~50 pills with porridge on an empty stomach.
- Coptis and Ass Hide Gelatin Formula 黃連阿膠元 황련아교원: It treats red-white dysentery and heat dysentery. Coptidis Rhizoma (黃連) 3 nyang and Poria Sclerotium (茯苓) 2 nyang. Powder these medicines, make into dough with Asini Corii Colla (阿膠) 1 nyang (mixed with water after being fried and powdered), and make into pills the size of a seed of a foxglove tree. Take 30~50 pills with porridge on an empty stomach.
- Stabilize Yang Decoction 固腸湯 고장탕: It treats red-white dysentery. Pericarpium Papaveris (罌粟殼) (fried after wetted with vinegar) 2 don, Paeoniae Radix (芍藥) 1.5 don, Angelica Gigantis Radix (當歸), Glycyrrhizae Radix (甘草) (baked) 7.5 pun each, Citrus Unshius Pericarpium (陳皮), Terminaliae Fructus (訶子), Zingiberis Rhizoma (乾薑) 5 pun each, Ginseng Radix (人參), and Aucklandiae Radix (木香) 3 pun each. Slice these medicines as 1 pack, decoct with water, and take on an empty stomach.
- Person of Integrity Maintain Organs Decoction 眞人養臟湯 진인양장탕: It treats many kinds of dysentery including red-white dysentery. Pericarpium Papaveris (罌粟殼) 1 don, Glycyrrhizae Radix (甘草) 9 pun, Paeoniae Radix (芍藥) 8 pun, Aucklandiae Radix (木香) 7 pun, Terminaliae Fructus (訶子) 6 pun, Cinnamomi Cortex (桂皮), Ginseng Radix (人參), Angelica Gigantis Radix (當歸), Atractylodis Rhizoma (蒼朮), and Myristicae Semen (肉豆蔲) 3 pun each. Slice these medicines as 1 pack, decoct in water, and take warm on an empty stomach.
10-4 Water-food Dysentery 水穀痢 수곡리
① Loose stools from deficiencies of the spleen and stomach qi leading to indigestion and may change into water-food dysentery.
② As swill diarrhea is also called water-food dysentery, refer to that chapter.
