06-1 Dampness Diarrhea 濕泄 습설
① As dampness diarrhea is soggy diarrhea, it is also called throughflux diarrhea. Feces comes out like pouring water, rumbling sounds are heard from the abdomen, and the body feels heavy without abdominal pain.
② In the Sir Zuo’s Commentary on the Spring and Autumn Annals (左傳), it is said, “If rain falls much, the disease appears,” which refers to this.
③ Because cold-dampness damages the spleen-stomach, it cannot ferment water and food, and throughflux diarrhea comes out as water. This is called soggy diarrhea. A Poria Calm Stomoch Decoction added with Alpiniae Katsumadaii Semen (草豆蔲) is commonly prescribed.
④ When the body aches from dampness diarrhea, Five Ingredients Powder with Poria added with Osterici Radix (羌活) and Atractylodis Rhizoma (蒼朮) should be prescribed.
⑤ Dampness diarrhea is where undigested water and food hampers the separation of the clear and the turbid.
⑥ Watery diarrhea without abdominal pain is dampness diarrhea. A Chuanxiong Medicated Leaven Pill should be prescribed.
⑦ When water and food flow out at once after drinking water due to severe thirst, Five Ingredients Powder with Poria (五苓散) should be prescribed.
⑧ A Drain Dampness Decoction, Guarding Life Decoction, and Five Ingredients Powder with Poria for Ten Thousand Diseases should be prescribed for throughflux diarrhea.
- Chuanxiong Medicated Leaven Pill 麴芎丸 국궁환: ① It treats efflux diarrhea caused by wind-dampness. Use the same amounts of Massa Medicata Fermentata (神麯), Cnidii Rhizoma (川芎), Atractylodis Rhizoma (蒼朮), and Aconiti Lateralis Radix Preparata (附子) (baked after wrapped with watery paper). Powder these medicines, combine with flour to make a paste, and make into pills the size of a seed of a foxglove tree. Take 30~50 pills with porridge on an empty stomach. ② In the Zuo’s Commentary on the Spring and Autumn Annals (左傳), it is said, “Massa Medicata Fermentata (神麯) and Cnidii Rhizoma (川芎) can remove dampness.” There is no case of diarrhea caused by spleen-dampness that cannot be cured. It can also cure swill diarrhea.
- Drain Dampness Decoction 瀉濕湯 사습탕: ① It treats throughflux diarrhea. Atractylodis Rhizoma (蒼朮) (baked) 3 don, Paeoniae Radix (芍藥) (baked) 2 don, Citrus Unshius Pericarpium (陳皮) (baked) 1.5 don, Saposhnikoviae Radix (防風) 1 don, Cimicifugae Rhizoma (升麻) 5 pun. Slice these medicines, decoct with water as one pack, and take it. ② This is an important prescription Liu Caochuang (劉草窓) used to treat throughflux diarrhea.
- Guarding Life Decoction 衛生湯 위생탕: It treats throughflux diarrhea. Ginseng Radix (人參), Atractylodis Rhizoma (蒼朮), Poria Sclerotium (白茯苓), Dioscoreae Rhizoma (山藥), Citrus Unshius Pericarpium (陳皮), Coicis Semen (薏苡仁), Alismatis Rhizoma (澤瀉) 1 don each, Coptidis Rhizoma (黃連), and Glycyrrhizae Radix (甘草) 5 pun each. Slice these medicines, decoct with water as one pack, and take it on an empty stomach.
- Five Ingredients Powder with Poria for Ten Thousand Diseases 萬病五苓散 만병오령산: It treats watery diarrhea without abdominal pain, rumbling sounds from the abdomen, and a fine pulse. Poria Sclerotium (茯苓), Atractylodis Rhizoma (蒼朮), Polyporus (猪苓), Alismatis Rhizoma (澤瀉), Dioscoreae Rhizoma (山藥), Citrus Unshius Pericarpium (陳皮), Atractylodis Rhizoma (蒼朮), Amomi Fructus (砂仁) (baked), Myristicae Semen (肉豆蔲) (baked while buried under an ash Fire), Terminaliae Fructus (訶子) (baked while buried under an ash Fire)) 8 pun each, Cinnamomi Cortex (桂皮), and Glycyrrhizae Radix (甘草) 5 pun each. Slice these medicines as 1 pack, put 2 mass of Zingiberis Rhizoma (乾薑), 1 Mume Fructus Praeparatum (烏梅), 1 dan of Junci Medulla (燈心), and decoct in water and take it on an empty stomach.
06-2 Wind Diarrhea 風泄 풍설
① An aversion to wind, spontaneous sweating, and bloody feces are signs of damage to wind in the spring and an invasion of dampness in the summer. As a result, diarrhea comes out suddenly.
② This diarrhea is from a wind pathogen entering the intestines.
③ A stomach Wind Decoction (胃風湯) should be prescribed when bloody feces is seen in the diarrhea.
④ When the wind pathogen is crouching inside, perspiration should be induced by a Cinnamomi and Ephedrae Decoction. (Prescription is mentioned in the chapter on Cold).
06-3 Cold Diarrhea 寒泄 한설
① An Order the Center Decoction (理中湯) added with Poria Sclerotium (茯苓), Magnoliae Cortex (厚朴) or Treat the Center Decoction (治中湯) (prescription is mentioned in the chapter on Cold) added with Amomi Fructus (砂仁) should be prescribed for cold diarrhea with the symptoms of aversion to cold, heaviness of body, bloating and pain in the abdomen with rumbling sounds, and feces of undigested food that looks like duck droppings.
② When a patient does not have thirst during diarrhea from cold, it is called apdang (鴨溏. Diarrhea like the feces of a duck) and it’s also called mokdang (鶩溏). It is diarrhea which is watery and white so it looks like duck droppings.
③ Cold diarrhea (it is also called mokdang (鶩溏)) is feces coming out like water. An Aconite Warm Center Decoction or Calm Stomach Powder (prescription is mentioned in the chapter on Five Viscera) should be prescribed with an Order the Center Decoction.
④ Four Pillars Powder or Six Pillars Powder should be prescribed for cold diarrhea.
⑤ The patient should take medicine early in the morning and in the evening for cold diarrhea. Eventhough warmed medicine is taken in the morning, the power of the medicine runs out and becomes ineffective by the evening, so it cannot fight with yin-qi of midnight. Therefore, the patient should take the medicine again before going to sleep.
- Aconite Warm Center Decoction 附子溫中湯 부자온중탕: It cures abdominal pain and diarrhea of undigested food and water from the attack of cold. Aconiti Lateralis Radix Preparata (附子) (baked after wrapped with damp paper), Zingiberis Rhizoma (乾薑) (baked after wrapped with damp paper) 1.5 don each, Ginseng Radix (人參), Atractylodis Rhizoma (蒼朮), Poria Sclerotium (白茯苓), Paeoniae Radix (芍藥), Glycyrrhizae Radix (甘草) (baked) 1 don each, Magnoliae Cortex (厚朴), Alpiniae Katsumadaii Semen (草豆蔲) (baked while buried under an ash Fire), and Citrus Unshius Pericarpium (陳皮) 6 pun each. Slice these medicines, decoct it as 1 pack, and take it on an empty stomach.
- Four Pillars Powder 四柱散 사주산: It treats cold and pain around the umbilicus, diarrhea, tinnitus, and dizziness due to deficiency and cold of the kidneys. Aucklandiae Radix (木香), Poria Sclerotium (白茯苓), Ginseng Radix (人參), and Aconiti Lateralis Radix Preparata (附子) (baked after wrapped with damp paper) 1.25 don each. Slice these medicines, decoct it as 1 pack, add 3 pieces of Zingiberis Rhizoma (乾薑), 2 Zizyphi Fructus (大棗), a little salt, and take it warm on an empty stomach.
- Six Pillars Powder 六柱散 육주산: It treats abdominal pain around the umbilicus and incessant diarrhea from deficient and cold kidneys. This is Four Pillars Powder combined with the same amounts of Terminaliae Fructus (訶子) and Myristicae Semen (肉豆蔲).
06-4 Summerheat Diarrhea 暑泄 서설
① A Mosla, Poria, and Polyporus Decoction added with Paeoniae Radix (芍藥) and Plantaginis Semen (車前子) or Cinnamomum, Poria, and Sweet Dew Drink should be prescribed for vexation, thirst, red urine, and watery diarrhea.
② Mosla Powder (prescription is mentioned in the chapter on Summer-heat) combined with Extraordinary Results Powder should be taken in summer when the patient has sudden watery diarrhea, darkened complexion, deficient pulse, vexation and thirst, and spontaneous sweating.
③ An Atractylodes Medicated Leaven Formula, Clear Six Pill, Akebiae Caulis and Poria Powder, Augment Base Powder, Six Harmonies Decoction, or Clear Summerheat and Augment qi Decoction [three prescriptions appear in the chapter on Summer-heat] should be prescribed for summerheat diarrhea.
- Mosla, Poria, and Polyporus Decoction 薷苓湯 유령탕: It treats when diarrhea is about to become dysentery in the summer. Alismatis Rhizoma (澤瀉) 1.2 don, Polyporus (猪苓), Poria Sclerotium (茯苓), Atractylodis Rhizoma (蒼朮), Elsholtziae Herba (香薷), Coptidis Rhizoma (黃連) (fried after soaked in the juice of Zingiberis Rhizoma (乾薑)), Lablab Semen (白扁豆), Magnoliae Cortex (厚朴) (processed) 1 don each, and Glycyrrhizae Radix (甘草) 3 pun. Slice these medicines, decoct it as 1 pack, and take it on an empty stomach.
- Cinnamomum, Poria, and Sweet Dew Drink 桂苓甘露飮 계령감로음: Cures polydipsia, diarrhea, or patient vomiting by intestinal convulsion which is caused by suffering from heatstroke. Talcum (滑石) 2 nyang, Poria Sclerotium (茯苓), Alismatis Rhizoma (澤瀉), Gypsum Fibrosum (石膏), Glauberite (寒水石), Glycyrrhizae Radix (甘草) 1 nyang each, Atractylodis Rhizoma Alba (白朮), Cinnamomi Cortex (桂皮), and Polyporus (猪苓) 5 don each. Powder these medicines fine, take it 2 don each with hot or cold water any time. It is better to put a little honey in it and it cures surprisingly when the patient takes it with water decocted with Zingiberis Rhizoma (乾薑).
- Atractylodes Medicated Leaven Formula 麴朮元 국출원: It treats sudden diarrhea from heatstroke. Massa Medicata Fermentata (神麯) (fried) and Atractylodis Rhizoma (蒼朮) (processed) the same amounts. Powder these medicines, make it into paste with flour and make into pills the size of a seed of a foxglove tree. Take 30~50 pills with thin rice gruel on an empty stomach.
- Clear Six Pill 淸六丸 청육환: It treats damp-heat diarrhea. Add half nyang of Oryzae Semen cum Monasco (紅麴) 3 nyang of Augment Base Powder. [Prescription appears in the chapter Summer-heat]. Powder these medicines, make it into paste with steamed old rice, and make into pills to the size of a seed of a foxglove tree. Take 50~70 pills with boiled water on an empty stomach.
- Akebiae Caulis and Poria Powder 通苓散 통령산: It treats diarrhea caused by summerheat-damp by separating the food and water and relieving vexation and heat. Alismatis Rhizoma (澤瀉), Atractylodis Rhizoma (蒼朮), Polyporus (猪苓), Poria Sclerotium (茯苓), Tetrapanacis Medulla (木通), Artemisiae Capillaris Herba (茵蔯), Dianthi Herba (瞿麥), and Plantaginis Semen (車前子) 1 don each. Slice these medicines, add 1 bundle of Junci Medulla (燈心), 10 Liriopis Tuber (麥門冬), and decoct as 1 pack and take.
06-5 Fire Diarrhea 火泄 화설
① Fire diarrhea is namely heat diarrhea. The patient will have a dry mouth and prefer cold beverages and will suffer from pain followed by diarrhea. When sudden diarrhea come out as sticky feces, a prescription of Coptis and Mosla Powder should be combined with Four Ingredients with Poria Powder and added with Paeoniae Radix (芍藥) and Gardeniae Fructus (梔子) (fried).
② Four Ingredients Powder with Poria for Ten Thousand Diseases is prescribed when abdominal pain leads to diarrhea that is like boiled water with heaviness on the bottom, red and difficult feces and urine, vexation, thirst, and a rapid pulse.
③ Abdominal pain followed by diarrhea with rumbling sounds result from fire. Prescribe Four Ingredients with Poria Powder added with Tetrapanacis Medulla (木通), Talcum (滑石), Scutellariae Radix (黃芩), and Gardeniae Fructus (梔子).
- Four Ingredients with Poria for Ten Thousand Diseases Powder 萬病四苓散 만병사령산: It treats heat diarrhea. Poria Sclerotium (茯苓), Atractylodis Rhizoma (蒼朮), Polyporus (猪苓), Alismatis Rhizoma (澤瀉), Atractylodis Rhizoma (蒼朮) (fried), Dioscoreae Rhizoma (山藥), Paeoniae Radix (芍藥) (fried), Gardeniae Fructus (梔子) (fried), Citrus Unshius Pericarpium (陳皮) 1 don each, and Glycyrrhizae Radix (甘草) (baked) 5 pun. Slice these medicines, add 1 Mume Fructus Praeparatum (烏梅), 1 bundle of Junci Medulla (燈心), decoct together as 1 pack, and take.
