06-6 Deficient Diarrhea 虛泄 허설

① When the patient is tired, spiritless, and diarrhea occurs right after eating food without abdominal pain, a Four Gentlemen Decoction should be taken with the addition of powdered Aucklandiae Radix (木香), Amomi Fructus (砂仁), Nelumbinis Semen (蓮肉), and old glutinous rice with sugared water on an empty stomach.

② Diarrhea occurring right after food entering the stomach or diarrhea of undigest food is deficient diarrhea. A Raise Yang and Eliminate Dampness Decoction should be prescribed.

③ When diarrhea of undigested food occurs right after food enters the stomach, along with a faint and fine pulse, Ginseng, Poria, Lotus Seeds, and White Atractylodes Powder should be prescribed.

④ When diarrhea is caused by qi-deficiency, double the amount of Atractylodis Rhizoma (蒼朮) in a Four Gentlemen Decoction, and raise qi up with the addition of Astragali Radix (黃芪), Cimicifugae Rhizoma (升麻), Bupleuri Radix (柴胡), and Saposhnikoviae Radix (防風).

⑤ For deficient diarrhea, Maintain Base Powder and an Augmented Four Gentlemen Decoction are both good.

  • Raise Yang and Eliminate Dampness Decoction 升陽除濕湯 승양제습탕: It treats diarrhea from qi deficiency with the symptoms of loss of appetite, tiredness, and spiritlessness. Atractylodis Rhizoma (蒼朮) 1.5 don, Cimicifugae Rhizoma (升麻), Bupleuri Radix (柴胡), Osterici Radix (羌活), Saposhnikoviae Radix (防風), Massa Medicata Fermentata (神麯), Alismatis Rhizoma (澤瀉), Polyporus (猪苓) 7 pun each, Citrus Unshius Pericarpium (陳皮), Hordei Vulgaris Fructus (麥芽) (fried), and Glycyrrhizae Radix (甘草) (baked) 5 pun each. Slice these medicines, decoct as 1 pack, and take on an empty stomach.
  • Ginseng, Poria, Lotus Seeds, and White Atractylodes Powder 參苓蓮朮散 삼령연출산: It treats diarrhea caused by qi-deficiency. Ginseng Radix (人參), Atractylodis Rhizoma (蒼朮), Poria Sclerotium (白茯苓), Dioscoreae Rhizoma (山藥), Nelumbinis Semen (蓮肉), Citrus Unshius Pericarpium (陳皮) 1 don each, Amomi Fructus (砂仁), Agastachis Herba (藿香), Terminaliae Fructus (訶子), Myristicae Semen (肉豆蔲), Zingiberis Rhizoma (乾薑) (baked after wrapped with damp paper), and Glycyrrhizae Radix (甘草) (baked) 5 pun each. Slice these medicines as 1 pack, put 1 Mume Fructus Praeparatum (烏梅), 1 bundle of Junci Medulla (燈心), and decoct with water and take.
  • Maintain Base Powder 養元散 양원산: It treats diarrhea and loss of appetite. After drying 1 doe of glutinous rice soaked in water for one night, fry completely over low heat and powder until fine. Put 1 nyang of Dioscoreae Rhizoma (山藥) powder and some of Piperis Nigri Fructus (胡椒) powder and mix well. Add a spoon of sugar to a half cup of this, mix with boiled water, and take every dawn. The taste of it is very good and it tonifies greatly. Also, it is better to add Nelumbinis Semen (蓮肉) and Euryales Semen (芡實) powder.
  • Augmented Four Gentlemen Decoction 加味四君子湯 가미사군자탕: It treats diarrhea caused by qi deficiency. Add 1 don each of Myristicae Semen (肉豆蔲) (baked while buried under an ash Fire) and Terminaliae Fructus (訶子) (baked after wrapped with damp paper) to a Four Gentlemen Decoction. Slice these medicines as 1 pack, put 3 pieces of Zingiberis Rhizoma (乾薑), 1 Zizyphi Fructus (大棗), decoct it with water, and take on an empty stomach.

06-7 Incessant Diarrhea 滑泄 활설

① Incessant diarrhea is diarrhea that does not stop for long or diarrhea that comes out directly as the anus is opened wide like a bamboo tube and the patient cannot stop it.

② Eight Pillars Powder should be prescribed when diarrhea comes out innumerably at day and night, or incessant diarrhea does not stop because the intestine-stomach is deficient and the pulse is sunken and fine.

③ A Stabilize yang Pill, Limonite Pill, Aucklandia Powder, or Replete Intestines Powder should be prescribed for incessant diarrhea.

④ A Perfect Pill should be prescribed when urine is accompanied by semen in the case of incessant diarrhea.

  • Eight Pillars Powder 八柱散 팔주산: It treats incessant diarrhea. Ginseng Radix (人參), Atractylodis Rhizoma (蒼朮), Myristicae Semen (肉豆蔲) (baked while buried under an ash Fire), Zingiberis Rhizoma (乾薑) (fried), Terminaliae Fructus (訶子) (baked after wrapped with damp paper), Aconiti Lateralis Radix Preparata (附子) (baked after wrapped with damp paper), Pericarpium Papaveris (罌粟殼) (fried with honey), and Glycyrrhizae Radix (甘草) (baked) 1 don each. Slice these medicines as 1 pack, put 2 pieces of Zingiberis Rhizoma (乾薑), 1 Mume Fructus Praeparatum (烏梅), 1 bundle of Junci Medulla (燈心) together, decoct with water, and take on an empty stomach.
  • Limonite Pill 禹餘粮丸 우여량환: It treats incessant diarrhea in patients with a deficiency and cold. Limonitum (禹餘粮) (heated), Halloysitum Rubrum (赤石脂) (heated), Fossilia Ossis Mastodii (龍骨), Piperis Longi Fructus (蓽撥), Terminaliae Fructus (訶子) (baked after wrapped with damp paper), Zingiberis Rhizoma (乾薑) (baked after wrapped with damp paper), Myristicae Semen (肉豆蔲) (baked while buried under an ash Fire), and Aconiti Lateralis Radix Preparata (附子) (baked after wrapped with damp paper) all the same quantities. Powder these medicines, make into paste with vinegar, and make into pills to the size of a seed of a foxglove tree. Take 70 pills with porridge on an empty stomach.
  • Aucklandia Powder 木香散 목향산: It treats incessant diarrhea from cold intestines with symptoms of undigested food, upper heat and lower cold, sores in the mouth, and weakening of the body. Aucklandiae Radix (木香), Psoraleae Semen (破故紙) (fried) 1 nyang each, Alpiniae Officinari Rhizoma (良薑), Amomi Fructus (砂仁), Magnoliae Cortex (厚朴) 7.5 don each, Paeoniae Radix (赤芍藥), Citrus Unshius Pericarpium (陳皮), Cinnamomi Cortex (桂皮), Atractylodis Rhizoma (蒼朮) 5 don each, Evodiae Fructus (吳茱萸), Piperis Nigri Fructus (胡椒) 2.5 don each, 4 Myristicae Semen (肉豆蔲) (baked buried in a fire turning to ashes), and 1 Arecae Semen (檳榔). Slice these medicines, cut 4 nyang of a pig’s liver, and scatter the slices 3 don each several times so it can permeate. Put all this in one bowl of the upper water of millet porridge, add a little vinegar, and boil it with the cover on. Add salt, 3 stems of Allii Fistulosi Bulbus (葱白), and Zingiberis Rhizoma (乾薑) (size of a bullet), and boil down until the water disappears. Then cool it and take it once on an empty stomach. It is okay even if the stool is soft at first. It is effective for long-lasting incessant diarrhea and cold dysentery. Make into porridge and take if thirst occurs.
  • Replete Intestines Powder 實腸散 실장산: It treats diarrhea caused by deficiencies and cold. Magnoliae Cortex (厚朴) (dried after soaked in Zingiberis Rhizoma (乾薑) juice) 1.5 don, Myristicae Semen (肉豆蔲) (baked while buried under an ash Fire), Terminaliae Fructus (訶子) (baked after wrapped with watery paper), Amomi Fructus (砂仁) (ground), Citrus Unshius Pericarpium (陳皮), Atractylodis Rhizoma (蒼朮), Poria Sclerotium (茯苓) 1 don each, Aucklandiae Radix (木香), and Glycyrrhizae Radix (甘草) 5 pun each. Slice these medicines as 1 pack, put 3 pieces of Zingiberis Rhizoma (乾薑), 2 Zizyphi Fructus (大棗), decoct in water, and take.
  • Perfect Pill 萬全丸 만전환: It treats chronic dysentery and diarrhea caused by cold. Halloysitum Rubrum (赤石脂), Zingiberis Rhizoma (乾薑) (baked after wrapped with watery paper) 1 nyang each, and Piperis Nigri Fructus (胡椒) 5 don. Powder these medicines, make into paste with vinegar, and make into pills the size of a seed of a foxglove tree. Take 5–7 pills with porridge on an empty stomach.

06-8 Swill Diarrhea 飱泄 손설

① Swill diarrhea is diarrhea of undigested food and beverages.

② Dinner is called son (飱). It is called swill diarrhea (diarrhea of dinner) since indigestion is more severe at dinner time.

③ In the Inner Classic (內經), it is said, “It becomes swill diarrhea if clear qi stays in the lower energizer.” In the Annotation it is said, “Clear qi is yang qi. Yang qi is heat qi so if heat qi is in the lower energizer, food and beverages are undigested and become swill diarrhea.”

④ Also it is said, “When chronic wind invades, it becomes intestinal wind or swill diarrhea.” The spleen and stomach digest and transform with mild energy. However, if clear qi moves downward or the wind pathogen stays long, it invades the stomach: the Wood is hurting the Earth. As a result, mild energy cannot move and change, so food and beverages come out in feces undigested. This is called swill diarrhea. Sometimes food is not digested when the patient eats so much that the intestine-stomach is damaged. Common people call this water-grain dysentery. Modified Aucklandia Powder is prescribed commonly.

⑤ Symptoms of swill diarrhea will disappear if the patient does not eat for one day. The reason for doing away with food is because of the weak stomach. If nothing is eaten, then the patient will not suffer from diarrhea. Afterwards, the yang qi of the patient should be raised with medicine like Maintain Base Powder or Eight Immortals Cake (八仙糕).

⑥ A Ledebouriella and Peony Decoction and Atractylodes and Ledebourellia Decoction should be prescribed for swill diarrhea.

⑦ Once there was a person suffering from swill diarrhea who had thundering sounds from the abdomen, loose bowels of undigested food, and difficulty in urination. The person was prescribed to take a Cinnamomi and Ephedrae Decoction with the addition of Zingiberis Rhizoma (生薑), and Zizyphi Fructus (大棗) three times in a row. Then the person perspired for a full day and recovered.

  • Modified Aucklandia Powder 加減木香散 가감목향산: ① It treats swill diarrhea and water-food dysentery. Aucklandiae Radix (木香), Alpiniae Officinari Rhizoma (良薑), Cimicifugae Rhizoma (升麻), Arecae Semen (檳榔), Ginseng Radix (人參), Atractylodis Rhizoma (蒼朮) 2.5 don each, Massa Medicata Fermentata (神麯) (fried) 2 don, Myristicae Semen (肉豆蔲) (baked while buried under an ash Fire), Evodiae Fructus (吳茱萸) (washed with boiled water), Zingiberis Rhizoma (乾薑) (baked after wrapped with watery paper), Citrus Unshius Pericarpium (陳皮), and Amomi Fructus (砂仁) 5 pun each. Powder these medicines rough, decoct 5 don each with water, and take on an empty stomach. ② This also cures intestinal wind and swill diarrhea.
  • Eight Immortals Cake 八仙糕 팔선고: It treats continuous diarrhea from deficiencies of the spleen and stomach. This is best for senile or pediatric use. Ponciri Fructus Immaturus (枳實) (fried with bran), Atractylodis Rhizoma (蒼朮) (fried with sand), Dioscoreae Rhizoma (山藥) 4 nyang each, Crataegi Fructus (山楂肉) 3 nyang, Poria Sclerotium (白茯苓), Citrus Unshius Pericarpium (陳皮) (fried), Nelumbinis Semen (蓮肉) 2 nyang each, and Ginseng Radix (人參) 1 nyang. Powder these medicines. Combine powdered nonglutinous rice 5 doe, glutinous rice 1.5 doe, and 3 geun of honey with medicine powder, put it in the steamer and steam it. Then dry under the sun and take (swallow by gargling with boiled water).
  • Ledebouriella and Peony Decoction 防風芍藥湯 방풍작약탕: It treats swill diarrhea with symptoms of bodily fever, stringy pulse, abdominal pain, and thirst. Saposhnikoviae Radix (防風), Paeoniae Radix (芍藥) 2 don each, and Scutellariae Radix (黃芩) 1 don. Slice these medicines, decoct in water, and take.
  • Atractylodes and Ledebourellia Decoction 蒼朮防風湯 창출방풍탕: It treats swills diarrhea from chronic wind in the person who cannot drink water and has diarrhea of undigested food and beverages. Atractylodis Rhizoma (蒼朮) 6 don, Ephedrae Herba (麻黃) 2 don, and Saposhnikoviae Radix (防風) 1 don. Slice these medicines as 1 pack, put 7 pieces of Zingiberis Rhizoma (乾薑), decoct in water, and take.

06-9 Phlegm Diarrhea 痰泄 담설

① Prescribe a Syzygii Flos Relieve Stomach Decoction with addition of Puerariae Radix (葛根), Atractylodis Rhizoma (蒼朮), and Massa Medicata Fermentata (神麯) for intermittent diarrhea in phlegm diarrhea with an increase and decrease in the amount of feces. Prescribe a Bluegreen Sea Pill for the excess case and Six Gentlemen Decoction for the deficient case.

② Prescribe a Syzygii Flos Relieve Stomach Decoction for Ten Thousand Diseases for the patient with a sunken and slippery pulse in phlegm diarrhea.

  • Bluegreen Sea Pill 海靑丸 해청환: It treats diarrhea from the accumulation of phlegm. Anodontae Caro (海粉) 1 nyang, Polygoni Tinctori Fructus (靑黛) 3 don, Scutellariae Radix (黃芩) 2 don, and Massa Medicata Fermentata (神麯) 5 don. Powder these medicines, make into paste with Massa Medicata Fermentata (神麯), and make into pills the size of a seed of a foxglove tree. Take 20~30 pills with boiled water on an empty stomach.
  • Two Matured Substances for Ten Thousand Diseases Decoction 萬病二陳湯 만병이진탕: It treats diarrhea from phlegm-dampness. Pinelliae Tuber (半夏), Citrus Unshius Pericarpium (陳皮), Poria Sclerotium (茯苓), Atractylodis Rhizoma (蒼朮), Atractylodis Rhizoma (蒼朮), Dioscoreae Rhizoma (山藥) 1 don each, Amomi Fructus (砂仁), Magnoliae Cortex (厚朴), Tetrapanacis Medulla (木通), Plantaginis Semen (車前子) (fried), and Glycyrrhizae Radix (甘草) (baked) 5 pun each. Slice these medicines as 1 pack, put 3 pieces of Zingiberis Rhizoma (乾薑), 1 Mume Fructus Praeparatum (烏梅), 1 bundle of Junci Medulla (燈心), decoct with water, and take.

06-10 Food Accumulation Diarrhea 食積泄 식적설

① Calm Stomach Powder (平胃散) with Cyperi Rhizoma (香附子), Amomi Fructus (砂仁), Amomi Tsao-ko Fructus (草果), Crataegi Fructus (山楂), and Hordei Vulgaris Fructus (麥芽) is prescribed when severe abdominal pain is relieved after diarrhea, the diarrhea smells of rotten eggs, and there is acid regurgitation when burping.

② Aucklandia and Amomum Calm Stomach Powder with the removal of Ponciri Fructus Immaturus (枳實) and the addition of Atractylodis Rhizoma (蒼朮) and Poria Sclerotium (白茯苓) is prescribed when severe abdominal pain is relieved after diarrhea with a string-like and slippery pulse.

③ Diarrhea from food accumulation is white in color.

④ When a patient has diarrhea from accumulation and stagnation, abdominal pain will be excruciating. Or the patient may feel abdominal distention, which is tight with pressure. The concoction should be digested with herbs like Massa Medicata Fermentata (神麯), Hordei Vulgaris Fructus (麥芽), and Crataegi Fructus (山楂).

⑤ When a patient has diarrhea of food-drink that has been stagnated for several days, it is called Dawn diarrhea. A Bitter Orange and Atractylodes Pill (枳朮丸) should be prescribed.
