16 Turbid, Red, or White Urine 赤白濁 소변이 붉거나 희면서 뿌연 것
① When urine is turbid, red, or white, a whirlpool shaped film floats on the surface like oil. The reflected light isn’t consistent and sometimes the whirlpool sinks down. The tangled and sunken residue looks like oil starch, unboiled rice water, flour starch, or reddish pus. All are caused by dampness heat inflicting internal damage. This is just like water becoming turbid on a hot day; diseases involving turbid urine must be caused by dampness-heat.
② That ancient sages said, “In the summer, the earth dries and water becomes turbid; in the winter, the earth hardens and water becomes clear.” When Water and Fire are in their proper places, the Earth hardens by itself and the flow becomes clear. Turbid urine is caused when there is deficient heat in the spleen and the kidney is deficient; as a result, pathogenic Earth invades Water.
③ In the Inner Classic, it says, “All turbid urine belongs to heat.”
④ Urine becomes turbid when dampness-heat of the spleen and stomach descend and permeate the bladder, making urine reddish, white turbid, and unclear. When blood is deficient and heat is severe, the urine is reddish and turbid; this is because diseases governed by the heart and the small intestine belong to Fire. On the other hand, when qi is deficient and the fever is weak, the urine is white and hazy; this is because diseases governed by the lungs and the large intestine belong to Metal.
⑤ Reddish, white, or turbid urine is caused when dampness-heat in the spleen makes the middle energizer unclear and turbid qi permeates the bladder.
⑥ Reddish and turbid urine means deficiency and heat in the heart caused by too much thinking, while the white and turbid one means deficiency and cold in the kidneys caused by sexual desire.
⑦ The primary cause of turbid urine is dampness-heat but phlegm and deficiency occasionally cause it as well. Reddish and turbid urine belongs to blood, while the white and turbid one belongs to qi. The therapy is the same as that for dysentery or vaginal discharge.
⑧ Turbid, red, or white urine of an obese person is mostly due to dampness-phlegm, so use a Syzygii Flos Relieve Stomach Decoction (二陳湯) with Atractylodis Rhizoma (蒼朮), Atractylodis Rhizoma Alba (白朮), Cimicifugae Rhizoma (升麻), and Bupleuri Radix (柴胡) or Arisaema, Pinellia, and Powdered Clam Pill. For a thin person, deficiency heat is usually the cause, so use a Four Substances Decoction with Anemarrhenae Rhizoma (知母) and Phellodendri Cortex (黃柏) or Pearl Powder Pill [the prescription is seen in the chapter on Essence] Ailanthi Pill.
⑨ Use Cinnabar, Aucklandia, Musk, and Cyperus Powder [the prescription is seen in the chapter on Spirit] and a Golden Lotus Pill for an exhausted heart caused by too much thinking. Use a Fish Poison Yam Separate and Clear Drink and Minor Cuscutae Seed Formula [the prescription is seen in the chapter on Deficiency Vexation] for exhausted kidneys caused by indulgences of sexual life. Use an Atractylodes Deal with Hardship Elixir, Four Stir-Fried Ingredients Elixir to Stabilize the True and Alum and Poria Formula for turbid urine caused by leaking essence, which is in turn due to uncollected spleen essence. Use a Tonify the Center to Augment the qi Decoction [the prescription is seen in the chapter on Internal Damage] for the drooped stomach qi.
⑩ Must add a Modified Pinctada Powder Pill for turbid urine.
⑪ The therapy would be for drying dampness, descending Fire, and lifting qi up. Use a Syzygii Flos Relieve Stomach Decoction with Atractylodis Rhizoma (蒼朮), Atractylodis Rhizoma Alba (白朮), Cimicifugae Rhizoma (升麻), Bupleuri Radix (柴胡), and Paeoniae Radix Alba (白芍藥).
⑫ Add Five Ingredients Powder with Poria (五苓散) [the prescription is seen in the chapter on Cold Damage] to a Four Gentlemen Decoction (四君子湯) [the prescription is seen in the chapter on Qi] and steep it for white and turbid urine. In addition, a Secret Essence Formula, Stabilize the Root Formula, Pinellia and Polyporus Pill, and Guide Out Red Decoction are all suitable.
⑬ Use an Augmented Heart-Clearing Drink and Clear Heart Lotus Seed Drink [the prescription is seen in the chapter on Wasting-thirst] for reddish and turbid urine. ⑭ Use an Euryales Formula [the prescription is seen in the chapter on Essence], Wonderful Response Pill. Cinnabar, Aucklandia, Musk, and Cyperus Powder [the prescription is seen in the chapter on Spirit.], Fire and Water Separate and Clear Drink and Mantidis Ootheca Powder for turbid, reddish, or white urine.
⑮ Consider turbid, reddish, or white urine and unbearable pain in the lower abdomen as cold-apoplexy. A Wine-cooked Angelica Pill [The prescription is seen in the chapter on Gynecology] of Li Dongyuan is the most profound.
- Arisaema, Pinellia, and Powdered Clam Pill 星半蛤粉丸 성반합분환: Used for white and turbid urine caused by dampness-heat. Anodontae Caro Pulvis (蛤粉) 2 nyang, Anodontae Caro Pulvis (蛤粉), Pinelliae Tuber (半夏) (soaked in Zingiberis Rhizoma (生薑) juice and dried together), Atractylodis Rhizoma (蒼朮) and Indigo Pulverata Levis (靑黛) 1 nyang each. Grind, boil Massa Medicata Fermentata (神麯) in the juice of Zingiberis Rhizoma (生薑), make into paste with starch, and make these into pills as big as a seed of the royal foxglove tree. Take 50 to 70 pills with Zingiberis Rhizoma (生薑)-steeped water on an empty stomach.
- Ailanthi Pill 樗柏丸 저백환: Used for turbid urine caused by dampness-heat and phlegm Fire. Phellodendri Cortex (黃柏) [descends Fire] 3 nyang, Ailanthi Radicis Cortex (樗根白皮) [stops diarrhea] 1 nyang, Indigo Pulverata Levis (靑黛) [opens stagnation], Zingiberis Rhizoma (乾薑) [collects the lungs and descends qi] 3 don each, Talcum (滑石) [eases urination], Anodontae Caro Pulvis (蛤粉) [enters the kidneys], and Massa Medicata Fermentata (神麯) [dries dampness] 5 don each. Grind, make into paste with starch, and make these into pills as big as a seed of the royal foxglove tree. Take 50 to 70 pills with boiled water on an empty stomach.
- Golden Lotus Pill 金蓮丸 금련환: Used for reddish and turbid urine caused by a damaged heart, which is in turn caused by thinking too much. Nelumbinis Semen (石蓮肉), Poria Sclerotium (白茯苓), Fossilia Ossis Mastodii (龍骨), Asparagi Tuber (天門冬), Liriopis Tuber (麥門冬), Thujae Semen (柏子仁), Angelica Gigantis Radix (當歸), Zizyphi Semen (酸棗仁), Fluoritum (紫石英), Polygalae Radix (遠志), Olibanum (乳香), and Fossilia Mastodi Dentis (龍齒) 1 nyang each. Grind, make into paste with honey, make into pills as big as a seed of the royal foxglove tree, and coat these with Cinnabaris (朱砂). Take 70 pills with warm wine or Zizyphi Fructus (大棗)-steeped water on an empty stomach.
- Fish Poison Yam Separate and Clear Drink 萆薢分淸飮 비해분청음: Used for white and turbid urine that’s clotted like starch. Acori Gramineri Rhizoma (石菖蒲), Linderae Radix (烏藥), Alpiniae Oxyphyllae Fructus (益智仁), Tokoro Rhizoma (萆薢), Poria Sclerotium (白茯苓) 1 don each, and Glycyrrhizae Radix (甘草) 5 pun. Chop into 1 package, add 1 handful of salt, steep with water, and take it on an empty stomach.
- Atractylodes Deal with Hardship Elixir 蒼朮難名丹 창출난명단: Used for turbid and continuous dribbling of urination caused by the leaking of spleen essence. It also cures weakness in the limbs and pain in the waist and back. Use medicine like Atractylodis Rhizoma (蒼朮) to collect the spleen essence. What does collecting the spleen essence mean? It means that essence is formed from grains. Atractylodis Rhizoma (蒼朮) (processed) 4 nyang, Foeniculi Fructus (茴香) (roasted), Meliae Fructs (川練子) 7.5 don each, Aconiti Tuber (川烏) (rolled in a damp paper and baked), Psoraleae Semen (破故紙) (roasted), Poria Sclerotium (白茯苓), and Fossilia Ossis Mastodii (龍骨) 1 nyang each. Grind into powder, make into paste with flour starch put in wine, make into pills to size of a royal foxglove tree, and coat these with Cinnabaris (朱砂). Take 50 to 70 pills with rice gruel on an empty stomach.
- Four Stir-Fried Ingredients to Stabilize the True Elixir 四炒固眞丹 사초고진단: ① Used for seminal emission caused by chronic deficiency of the original viscera. It also cures white and turbid urine, the five strangury diseases, the seven lower abdominal colic, flooding and spotting, and vaginal discharge of women. Chop 1 geun of Atractylodis Rhizoma (蒼朮) into quarters. Roast one with Foeniculi Fructus (茴香) and Sal (靑鹽) 1 nyang each, one with Linderae Radix (烏藥) and Meliae Fructs (川練子) 1 nyang each, one with Zanthoxyli Pericarpium (川椒) and Psoraleae Semen (破故紙) 1 nyang each, and one with wine and vinegar until Atractylodis Rhizoma (蒼朮) turns yellow. Grind only Atractylodis Rhizoma (蒼朮), make into paste with flour starch, and make these into pills as big as a seed of the royal foxglove tree. Take 30 to 50 pills on an empty stomach. A man takes it with wine, while a woman takes it with vinegar-mixed water.
② As one cannot control sexual desire on being drunken and full, the spleen and the kidneys are damaged and the spleen restrains the kidneys. It is called “Togeuksu” (土克水, Earth restrains Water.) When the spleen pulse is surging and rapid and the kidney pulse is faint and rough, the urine is turbid and golden-yellow in color like Gardeniae Fructus (梔子) juice. When the urine is sunken, something like lime covers the bottom of a urinal tub, or a little blood is clotted inside. Use this medicine to tonify the spleen and stomach.
- Alum and Poria Formula 蠟苓元 납령원: It is also called Powerful Happiness Formula (威喜元). Used for white, turbid, and dribbling of urination caused by dampness pathogenic qi in the kidneys and not being able to hold essence qi. It also cures the white ooze and white vaginal discharge of a woman, as well as urination that’s like un-boiled rice water. Chop snow-white Poria Sclerotium (茯苓) 4 nyang, put it with chopped Polyporus (猪苓) 2.5 don in a porcelain and boil it 20 times with water. Remove Polyporus (猪苓), dry Poria Sclerotium (茯苓), and grind this. Make into paste with melted Cera Flava (黃蠟) 4 nyang and make these into pills as big as a bullet. Take it with Zizyphi Fructus (大棗)-steeped water and chew well. Use this until urine turns clear.
- Modified Pinctada Powder Pill 加減珍珠粉丸 가감진주분환: ① Used for turbid, reddish or white urine, and also cures white ooze. Phellodendri Cortex (黃柏) [Half raw and half roasted.], Anodontae Caro Pulvis (蛤粉) 3 nyang, Talcum (滑石) 2 nyang, Ailanthi Radicis Cortex (樗根白皮) 1 nyang, Indigo Pulverata Levis (靑黛) and Zingiberis Rhizoma (乾薑) (roasted until it turns brown) 5 don. Grind into powder, roast Massa Medicata Fermentata (神麯), make into paste with starch, and make these into pills as big as a seed of the royal foxglove tree. Take 70 to 100 pills with wine on an empty stomach.
② Phellodendri Cortex (黃柏) lowers deficiency Fire and removes dampness-heat. The salty taste of Anodontae Caro Pulvis (蛤粉) tonifies the kidneys. Talcum (滑石) eases urination. Ailanthi Radicis Cortex (樗根白皮) dries dampness-heat well. Indigo Pulverata Levis (靑黛) opens the stagnation and descends Fire. Zingiberis Rhizoma (乾薑) collects and lowers lung qi and makes yin blood. Process it with salt and bake it until it become dark brown.
- Secret Essence Formula 秘精元 비정원: ① Used for white and turbid urine caused by a deficiency in the lower energizer and coldness in the urinary bladder. Ostreae Testa (牡蠣) (heated), Cuscutae Semen (兎絲子) (processed with wine), Fossilia Ossis Mastodii (龍骨) (crude), Schisandrae Fructus (五味子), Allii Tuberosi Semen (韭子) (roasted), Poria Sclerotium (白茯苓), Halloysitum Album (白石脂) (heated), and Mantidis Oőtheca (桑螵蛸) (soaked in wine and baked) all the same amounts. Grind into fine powder, make into paste evenly with starch, put in wine, and make it into pills as big as a seed of the royal foxglove tree. Take 70~90 pills with boiled salt water on an empty stomach.
② It is also called a Stabilize the Essence Formula (固精元).
- Stabilize the Root Formula 固本元 고본원: Used for turbid urine. Prepare Glycyrrhizae Radix (甘草) (baked) 3 nyang, Polyporus (猪苓) 2.5 nyang, Nelumbinis Stamen (蓮花蘂), Coptidis Rhizoma (黃連) 2 nyang each, Poria Sclerotium (白茯苓), Amomi Fructus (砂仁), Alpiniae Oxyphyllae Fructus (益智仁), Pinelliae Tuber (半夏) (soaked in Zingiberis Rhizoma (生薑) juice and dried), and Phellodendri Cortex (黃柏) (roasted) 1 nyang. Grind into powder, make into paste with steamed rice-cake soaked in boiled water, and make it into pills as big as a seed of the royal foxglove tree. Take 50 to 70 pills with warm wine on an empty stomach.
- Pinellia and Polyporus Pill 半苓丸 반령환: Used for turbid urine. Namely, a Polyporus Pill in the chapter of Essence.
- Guide Out Red Decoction 導赤湯 도적탕: Used for urination that’s like unboiled rice water. One is cured before taking it twice. Akebiae Caulis (木通), Talcum (滑石), Phellodendri Cortex (黃柏), Poria Sclerotium Rubra (赤茯苓), Rehmanniae Radix Crudus (生地黃), Gardeniae Fructus (梔子仁), Glycyrrhizae Radix (甘草) (small roots) 1 nyang each, Aruantii Fructus Immaturus (枳殼) and Atractylodis Rhizoma Alba (白朮) 5 pun each. Chop these into 1 package, steep with water, and take it on an empty stomach.
- Augmented Heart-Clearing Drink 加味淸心飮 가미청심음: Used for reddish and turbid urination caused by heart heat. Nelumbinis Semen (蓮肉) and Poria Sclerotium (白茯苓) 1.5 don each, Alpiniae Oxyphyllae Fructus (益智仁), Liriopis Tuber (麥門冬), Polygalae Radix (遠志), Ginseng Radix (人參) 8 pun each, Acori Gramineri Rhizoma (石菖蒲), Plantaginis Semen (車前子), Atractylodis Rhizoma Alba (白朮), Alismatis Rhizoma (澤瀉) and Glycyrrhizae Radix (甘草) 5 pun each. Chop these into 1 package, add 20 stems of Junci Medulla (燈心), steep with water, and take it on an empty stomach.
- Wonderful Response Pill 妙應丸 묘응환: Used for turbid, reddish or white urination. Cuscutae Semen (兎絲子) (processed with wine), Mantidis Oőtheca (桑螵蛸) (soaked in wine and baked), Meliae Fructus (川練肉) 5 pun each, Ostreae Testa (牡蠣) (baked) 3 don, Fossilia Ossis Mastodii (龍骨), Cinnabaris (辰砂), Acori Gramineri Rhizoma (石菖蒲), Poria Sclerotium (白茯苓), Alpiniae Oxyphyllae Fructus (益智仁), Nelumbinis Semen (蓮肉). and Amomi Fructus (砂仁) 2.5 don each. Grind into powder, make into paste with starch cooked with Dioscoreae Rhizoma (山藥), and make these into pills as big as a seed of the royal foxglove tree. Take 50 pills with water steeped with Ginseng Radix (人參) and Zizyphi Semen (酸棗仁) at noon and 50 pills with nonglutinous rice-boiled water before going to bed.
- Chicken Clear Formula 雞淸元 계청원: Widely used for turbid urine. Grind a big, crude Pinelliae Tuber (半夏) into powder, make into paste with the white of an egg, and make it into pills as big as a seed of the royal foxglove tree. Dry it a little and bake it with Polyporus (猪苓) over a weak fire until the pills split open. Never use Polyporus (猪苓) powder. Save in a bowl and take 30 to 50 pills with Poria Sclerotium (白茯苓)-steeped water on an empty stomach.
- Fire and Water Separate and Clear Drink 水火分淸飮 수화분청음: Used for turbid, reddish or white urine. Prepare Poria Sclerotium Rubra (赤茯苓) 1 don, Alpiniae Oxyphyllae Fructus (益智仁), Tokoro Rhizoma (萆薢), Acori Gramineri Rhizoma (石菖蒲), Polyporus (猪苓), Plantaginis Semen (車前子), Alismatis Rhizoma (澤瀉), Atractylodis Rhizoma Alba (白朮), Citrus Unshius Pericarpium (陳皮), Aruantii Fructus Immaturus (枳殼), Cimicifugae Rhizoma (升麻) 7 pun each, and Glycyrrhizae Radix (甘草) 5 pun. Chop these into 1 package, steep these with wine and water, half and half, and take it on an empty stomach.
- Mantidis Ootheca Powder 桑螵蛸散 상표초산: The symptoms of frequent urination that’s turbid like thick unboiled rice water, a vague memory, and weak body are due to an overly indulgent sexual life. Prepare Mantidis Oőtheca (桑螵蛸) (dressed with salted water and steamed), Polygalae Radix (遠志) (soaked in Zingiberis Rhizoma (生薑) juice and dried), Acori Gramineri Rhizoma (石菖蒲) (dipped in salted water and roasted), Fossilia Ossis Mastodii (龍骨), Ginseng Radix (人參), Hoelen cum Radix (白茯神), Angelica Gigantis Radix (當歸) (washed in water), HAmydae Carapax (鱉甲) (dipped in vinegar and baked) 5 don each, and Glycyrrhizae Radix (甘草) (baked) 2.5 don. Grind these into powder, mix it with water steeped with Ginseng Radix (人參), Poria Sclerotium (茯苓), and Mori Cortex (桑白皮) and take it before going to bed. Mori Cortex (桑白皮) makes water flow and return to the kidneys with Mantidis Oőtheca (桑螵蛸).
17 Parasitic Diseases and White Ooze 蠱病白淫 고병백음
① The Inner Classic says that, “A disease of the spleen that has been delivered to the kidneys is called a genital disease with abdominal mass. Stagnation of heat in the lower abdomen and urination with something white is called a parasitic disease.” An Annotation says, “The kidney meridian goes through the spine, touches the kidneys, and connects to the bladder. Hence there is the stagnation of heat and pain in the lower abdomen and urination with white fluid. The stagnation of heat inside burning fat and flesh damages and weakens the inner body every day; just like an insect eating through.” That is why one gets weaker whenever having sex.
② When the spleen gets pathogenic wind and transfers it to the kidneys, pathogenic heat burns inside. The symptoms will be the stagnation of heat and pain in the lower abdomen and urination with white fluid. It is called a parasitic disease. This is because pathogenic heat is stagnated inside and one is not able to protect genuine essence. Use a Cistanche Pill.
③ The Inner Classic says, “When one cannot achieve what one wants and keeps thinking about sex or indulges in a sex life, one’s penis will drop down and this leads to the sinew wilting and white ooze.” White ooze is the dribbling of white liquid that’s like sperm. Use a Pinellia and Polyporus Pill (半苓丸). [The prescription is seen later in the chapter].
④ Dairen (戴人) said, “Enuresis, uroschesis, and ischuria, impotence, the urinary impediment, spermatorrhea, and white ooze all belong to the genital diseases of men. Amenorrhea caused by dried blood, heat in the waist and knees, leg flaccidity, a dried throat, uroschesis and ischuria, a fixed or moving lump, a projected anterior yin, and hemorrhoids in the posterior yin belong to the genital diseases of women. However, in the case of women, they are not called genital diseases but called abdominal mass.
- Cistanche Pill 肉蓯蓉丸 육종용환: Used for parasitic diseases. Cistanchis Herba (肉蓗蓉), Poria Sclerotium (白茯苓), Astragali Radix (黃芪), Alismatis Rhizoma (澤瀉), Ostreae Concha (牡蠣粉), Schisandrae Fructus (五味子), Fossilia Ossis Mastodii (龍骨), and Angelica Gigantis Radix (當歸) 1 nyang each. Grind into powder, make into paste with honey, and form it into pills as big as a seed of the royal foxglove tree. Take 30 pills with wine on an empty stomach.
18 Urinary Bladder Blockage Syndrome 脬痺證 포비증
① The Inner Classic says, “The symptoms of urinary bladder impediment occur when the lower abdomen and bladder are pressed. There is pain and the sensation of warm water pouring into the bladder, and dysuria and clear snivel are also seen.”
② The bladder is a house of fluid-humor. Urine may come out through qi transformation. However, when wind, cold, and dampness pathogenic qi attack the urinary bladder, it fails to transform qi and cannot send urine out, so the urinary bladder gets filled up and urine cannot come out. Because the bladder meridian connects from the crown to the brain, snivel comes out when brain qi goes down to the nose.
③ The urinary bladder impediment is a kind of cold strangury and the causes are wind, cold, and dampness. Use a Morinda Pill and Warm Kidneys Decoction.
- Morinda Pill 巴戟丸 파극환: Used for difficult urination and dysuria. Morindae Radix (巴戟) 1.5 nyang, Mantidis Oőtheca (桑螵蛸) (baked with bran), Polygalae Radix (遠志) (soaked in Zingiberis Rhizoma (生薑) juice and dried), Rehmanniae Radix (生乾地黃) (washed in wine), Dioscoreae Rhizoma (山藥), Aconiti Lateralis Radix Preparata (附子) (rolled in a damp paper and baked), Dipsaci Radix (續斷), Cistanchis Herba (肉蓗蓉) (soaked in wine) 1 nyang each, Eucommiae Cortex (杜冲) (baked), Dendrobii Herba (石斛), Cervi Parvum Cornu (鹿茸), Fossilia Ossis Mastodii (龍骨), Cuscutae Semen (兎絲子) (processed with wine), Schisandrae Fructus (五味子), Corni Fructus (山茱萸), and Cinnamomi Cortex Spissus (肉桂) (roundly rolled one with good flavor) 3 don each. Grind, make into paste with honey, and make it into pills as big as a seed of the royal foxglove tree. Take 50 to 70 pills with wine on an empty stomach.
- Warm Kidneys Decoction 溫腎湯 온신탕: Used for difficult urination caused by a urinary bladder impediment. Poria Sclerotium Rubra (赤茯苓), Atractylodis Rhizoma Alba (白朮), Alismatis Rhizoma (澤瀉), and Zingiberis Rhizoma (乾薑) (rolled in a damp paper and baked) 1.25 don each. Chop into 1 package, steep with water, and take it on an empty stomach.
