“Clinical Differentiation by Key Signs: Neck Pain, Upper Back Pain, and Thirst in Jiuwei-Qianghuo-Tang”

By Ju Bong Kang KMD

[Acts and Diagnosis]

This formula treats symptoms such as chilliness, fever, head and neck stiffness, body aches, eye pain, nasal congestion, cough, and chest oppression caused by wind-cold and dampness.

Renshen in Renshen-Baidu-San boosts qi, helps expel external pathogens, and replenishes energy lost through diaphoretics. If the pulse is strong and the fever severe, Renshen can be removed and replaced with Jingjie and Fangfeng, forming Jingfang-Baidu-San. This use of Renshen parallels its role in Xiao-Chaihu-Tang.

In practice, Renshen-Baidu-San or Jingfang-Baidu-San is often combined with Shuanghuo-Tang to form Shuangbai-Tang, which is used broadly for heat syndromes caused by wind-cold. When treating fever, headache, body aches, or cough, formulas from the Shang Han Lun must be chosen based on pattern differentiation—superficial versus interior, deficiency versus excess. Shuangbai-Tang is commonly used in uncomplicated exogenous infections as it covers all pattern types.

Qianghuo dispels wind, dampness, and cold, and is especially effective for upper body pain (e.g., headaches, joint pain) caused by Taiyang invasion.

Duhuo also expels wind and dampness but is gentler and more effective for alleviating lower body pain and numbness (e.g., the waist, hips, legs, and knees).

Chaihu (minister drug) targets Shaoyang disorders, such as fever, chills, sore throat, and bitter taste, resolving exterior wind-cold or wind-heat conditions.

Chuanxiong promotes qi and blood circulation, dispels wind, and enhances relief from pain and fever, supporting Qianghuo and Duhuo.

Renshen, Jiegeng, Qianhu, Zhike, and Fuling are adjuvant drugs.
Renshen boosts qi and body fluids, enhancing immunity and compensating for the loss caused by diaphoretic herbs.

Jiegeng eliminates phlegm, relieves cough, clears heat, and enhances the effects of Qianghuo, Duhuo, Chaihu, and Qianhu, thereby improving symptoms of sore throat, cough, and asthma.
Qianhu expels wind-cold or wind-heat from the Lung, eliminates phlegm, and descends rebellious qi, relieving cough, fever, sore throat, and chest fullness.
Zhike resolves phlegm and food retention, alleviating chest and abdominal fullness, vomiting, and pain from digestive stagnation.
Fuling promotes urination and drains dampness, removing phlegm-retention from wind-cold or wind-dampness, and restores symptoms like dysuria, edema, palpitations, anxiety, and insomnia.
Shengjiang disperses wind-cold from the exterior and warms the middle, relieving vomiting caused by damp phlegm in the digestive tract.

Gancao and Bohe are guiding drugs, in addition to their adjuvant functions.
Gancao assists Renshen in tonifying qi, harmonizes the formula, and acts as a guiding herb.
Bohe expels wind-heat and guides herbs to the surface with its light and floating nature.
Renshen-Baidu-San is used when symptoms such as fever, headache, neck pain, eye pain, cough, and limb pain appear, and the pulse is floating, rapid, but weak.
Jingfang-Baidu-San is used for similar symptoms when the fever is more intense, and the pulse is floating, rapid, full, and tense.

Jiuwei-Qianghuo-Tang and Renshen-Baidu-San both treat fever, headache, lack of sweating, and floating pulse from external infection. However, Jiuwei-Qianghuo-Tang is marked by neck/shoulder pain and thirst, while Renshen-Baidu-San more often addresses rashes, boils, itching, and eczema.
