
For Degenerative OA, Use Heart Gate and Fire Master

Degenerative knee pain and osteophytes targeted Tung combinations often yield strong results. Below are clinic-ready sets, BL40 bleeding guidance, and “close-to-bone” technique notes.
▷Shoulder Center (44.06), PC6 (contralateral), Fire Master (66.04, ipsilateral): Use for swollen, painful, tight, or deformed knees. Add two points 0.5 cun beside the middle fingernail for stronger effect. If pain is in the popliteal fossa, add BL40/UB40.
▷Four Flower Center (77.09)+Four Flower Quasi (77.10): Needle close to the bone, then combine with PC6, Heart Gate (33.12), and Fire Master (66.04, ipsilateral). Excellent for degenerative OA, knee and heel spurs, and reducing bony hypertrophy. Also benefits lumbar and cervical osteophytes.
• Technique: needle the Four Flower pair against bone. For OA, bleed Three Metal (DT.07).
• Course: after several sessions, osteophyte-related pain often decreases.
• Signatures: ① Knee spur-worse descending stairs, better ascending. ②Heel spur-worst on first steps, improves, then worsens after long walking.
• Tip: for cervical/lumbar/knee/heel osteophytes, prick BL40 for strong effect.
▷Spirit Bone (22.05; Ling Gu): Needle close to bone for the “treat bone with bone” effect. Also effective for sciatica from lumbar herniation and bony finger swelling in RA.
▷Heart Gate (33.12, contralateral) + Fire Master (66.04, ipsilateral): For OA due to osteophytes—sharp pain on standing or after a misstep; also post-slip injuries. Highly effective for degenerative cases.
▷Heart Gate (33.12, contralateral)+Fire Knee (11.16, ipsilateral): For deforming OA, heavy/aching knees with crepitus and medial pain (SP9–LV8). Often linked to synovial fluid depletion (jin-ye deficiency), where Heart Gate is especially effective.
• Fire Knee (11.16): also treats palpitations, chest oppression, and Heart Fire-related pain. For shoulder, knee, and frozen shoulder along the SI channel, needle ipsilaterally. → Location: ~0.1 cun posterior to SI1.
▷Gallbladder (11.13)+Heart Knee (11.09): Simple, reliable first-line knee pain set.
• GB (11.13): useful for inner/outer knee-eye pain and sciatica.
• For bilateral sciatica, combine with Cha San.
▷HT8 (sedate), SP3 (sedate), ST36 (tonify): Powerful systemic set for multi-joint issues.
• HT8 & SP3 clear Heart excess; SP3 resolves dampness.
• For knee/back pain: sedate SP3, tonify ST36 to dry dampness.
• For thin/dry patients, avoid over-drying.
Clinical Notes & Safety
- “Close to the bone” = periosteal-adjacent depth; insert slowly within tolerance.
• BL40 bleeding: light prick-bleeding at crease center; maintain asepsis and check for anticoagulant use.
• DT.07 bleeding: ensure patient suitability (coagulation and consent) beforehand.
