14 Eight Types of Strangury Diseases 淋病有八 8가지 임병

There are also the unctuous, sandy, and cold stranguries in addition to the five stranguries. There are eight kinds in total.

14-1 Unctuous Strangury 膏淋 고림

① Unctuous strangury involves oily urination and the inside of the penis feeling painful and rough.

② The urine is turbid and things looking like clotted oil float.

③ Use a Cervi Cornus Degelatinatum Pill, Processed Urine Deposit Formula, Lygodium Fern Spores Powder, and Musk Powder for Infants for unctuous strangury.

④ A boy had suffered from unctuous strangury for 3 years. There was no improvement with medicine, food, acupuncture, or moxibustion. A doctor looked into it and said, “This may be a throat-anus-genitals syndrome or parasitic diseases. It is also called white ooze actually not caused by deficiency but caused by heat in the lower abdomen.” After vomiting 3 doe of phlegm and having diarrhea several times, he soon recovered.

  • Cervi Cornus Degelatinatum Pill 鹿角霜丸 녹각상환: Used for yellow, reddish, white, or black unctuous strangury-like oil, dribbling, and painful urination. Cornu Cervi Degelatinatum (鹿角霜), Poria Sclerotium (白茯苓), Depositum Urinae Praeparatum (秋石) (refined) all the same amount. Grind into powder, make into paste with starch, and make these into pills as big as a seed of the royal foxglove tree. Take 50 pills with a rice gruel.
  • Processed Urine Deposit Formula 秋石元 추석원: Used for yellow, reddish, white, or black unctuous strangury coming out like oil or honey. It occurs when turbid qi interferes with the clear one and essence is scattered. Poria Sclerotium (白茯苓) 1 nyang, Mantidis Oőtheca (桑螵蛸) (baked), Cervi Cornus Colla (鹿角膠珠), and Depositum Urinae Praeparatum (秋石) 5 don each. Grind, make into paste with starch cooked with rice cake, and make these into pills as big as a seed of the royal foxglove tree. Take 50 pills with water boiled with Ginseng Radix (人參) on an empty stomach.
  • Lygodium Fern Spores Powder 海金沙散 해금사산: Used for unctuous strangury. Lygodii Spora (海金沙), Talcum (滑石) 1 nyang each, and Glycyrrhizae Radix (甘草) 2.5 don. Grind, mix 1 don each with water boiled with Liriopis Tuber (麥門冬), and Junci Medulla (燈心) and take it.
  • Musk Powder for Infants 香兒散 향아산: Used for symptoms that have been mentioned above in the blood, sandy, and unctuous strangury chapters, plus piercing pain. Pound Moschus (麝香) having good quality 5 pun and 1 root of Allii Fistulosi Bulb (葱白) and the squeeze juice from it. Grind Rungia Pectinata Nees (孩兒茶) 3.5 don and Succinum (琥珀) 2.5 pun into powder, put these into the boiled water, add the juice, and take it on an empty stomach. It may be wonderfully efficacious.

14-2 Sand Strangury 沙淋 사림

① Sandy strangury involves sand in the penis causing pain and unpleasant sensations; the pain stops when there is sudden dysuria followed by the sand coming out.

② The sand coming out with urination and sinking to the bottom of a chamber pot is due to condensation of the fluid-humor by the yin Fire of the bladder. The light one becomes sandy strangury, while the heavy one becomes stone strangury.

③ Use Two Spirits Powder, Otolith Powder, Polygonum Powder, or Amber Powder for sandy strangury.

  • Two Spirits Powder 二神散 이신산: Used for sudden pain caused by sandy and stone strangury. Lygodii Spora (海金沙) 7.5 don, Talcum (滑石) 5 don. Grind these into powder. Add some honey to water steeped with Akebiae Caulis (木通), Liriopis Tuber (麥門冬), and Plantaginis Herba (車前草), mix these with medicinal powder 2 don and take it.
  • Otolith Powder 魚石散 어석산: Used for pain inside the penis caused by sandy and stone strangury. Five pairs of bones inside the head of a yellow croaker (heated and ground) and Talcum (滑石) 5 don. Grind these into powder and take twice. By taking it with water steeped with Akebiae Caulis (木通), one may be cured as the sand comes out.
  • Polygonum Powder 苦杖散 고장산: Used when sandy and stone strangury causes clunking sounds in the urine pot whenever urinating; it also cures unbearable pain. Slice Polygoni Cuspidati Radix (杜牛膝) [or Polygoni Cuspidati Radix (虎杖根)], mix each 1 hop [or 1 nyang] with 5 glasses of water, and steep until 1 glass remains. Remove the leftovers, mix some grindings of Moschus (麝香) and Olibanum (乳香), and then take it.
  • Amber Powder 琥珀散 호박산: Used for sandy and stone strangury. Succinum (琥珀), Talcum (滑石) 2 don each, Akebiae Caulis (木通), Angelica Gigantis Radix (當歸), Aucklandiae Radix (木香), Curcumae Radix (鬱金), and Polygoni Avicularis Herba (萹蓄) 1 don each. Grind these, mix each 3 don with water steeped with reed leaves, and take it on an empty stomach. Without reed leaves, use Phyllostachys Folium (竹葉) instead.

14-3 Cold Strangury 冷淋 냉림

① Cold strangury involves shivering caused by a chill first, followed by difficult, and frequent urination, and there is swelling and pain inside the urethra.

② Cold strangury is due to cold qi. The symptoms involve urination with shivering. Cold qi fights against healthy qi, so when cold qi wins, there is a strangury disease with shivering. If healthy qi wins, the shivering disappears, and urine comes out.

③ Use an Eight Ingredients Pill [the prescription is seen in the chapter on Deficiency Vexation], Raw Aconite Decoction, and Aucklandia Decoction for cold strangury.

  • Raw Aconite Decoction 生附湯 생부탕: Used when urination is difficult and painful and shivering getting worse. Aconiti Lateralis Radix Preparata (附子) (crude), Talcum (滑石) 7 pun each, Akebiae Caulis (木通), Pinelliae Tuber (半夏) (processed), and Dianthi Herba (瞿麥) 1.2 don each. Chop these to 1 package, mix 7 pieces of Zingiberis Rhizoma (生薑), 20 stems of Junci Medulla (燈心), and a half spoon of honey, steep with water and take on an empty stomach.
  • Aucklandia Decoction 木香湯 목향탕: Used for the dribbling of urination and a cold body. Akebiae Caulis (木通), Aucklandiae Radix (木香), Angelica Gigantis Radix (當歸), Paeoniae Radix Alba (白芍藥), Citri Unshius Pericarpium Immaturus (靑皮), Foeniculi Fructus (茴香), Arecae Semen (檳榔), Alismatis Rhizoma (澤瀉), Citrus Unshius Pericarpium (陳皮), Glycyrrhizae Radix (甘草) 7 pun each, and Cinnamomi Cortex Spissus (肉桂) 3 pun. Chop these to 1 package, add 5 pieces of Zingiberis Rhizoma (生薑), steep with water, and take on an empty stomach.

15 Common Medicinals for Treating Strangury 諸淋通治 모든 임병을 두루 치료하는 약

① There isn’t just one cause for strangury. It is due to the overindulgence of sexual relations, anger, medicated wine, or greasy food. There may be the rise of Fire due to an yin deficiency caused by overindulgence in sexual life, qi movement making Fire caused by anger, and dampness-heat caused by medicated wine and greasy food. When accumulated heat has been there long enough, it is stagnated in the lower energizer and causes dribbling and painful urination. At first, it becomes heat and blood strangury. As time goes on, fluid and humor gets boiled down and it becomes turbid and thick; it later becomes like sand and stone. By scattering the heat and easing urination, only then can heat and blood strangury be cured. For unctuous, sandy and stone strangury, one must scatter stagnation, circulate qi, break static blood and nourish yin. Ancient prescriptions break stagnation with Curcumae Radix (鬱金) and Succinum (琥珀), circulate qi with Citri Unshius Pericarpium Immaturus (靑皮) and Aucklandiae Radix (木香), break static blood with Typhae Pollen (蒲黃) and Achyranthis Radix (牛膝) and nourish yin with Phellodendri Cortex (黃柏) and Rehmanniae Radix Crudus (生地黃). Dongyuan used Citri Unshius Pericarpium Immaturus (靑皮) and Phellodendri Cortex (黃柏) for the lower abdomen. Citri Unshius Pericarpium Immaturus (靑皮) opens liver qi and Phellodendri Cortex (黃柏) nourishes the kidneys. The reason is that the lower abdomen is the part of the liver and kidneys.

② For all strangury, use a Drain Kidneys Decoction, Turmeric and Coptis Pill, Three Factors Amber Powder, Pyrrosia Powder, Five Stranguries Powder, Rice Paper-Plant Pith Decoction, Amber Powder [the prescription is seen later in the chapter], Two Spirits Powder [the prescription is seen later in the chapter], Definite Effect Powder, Lygodium Fern Spores Powder, Danliao’s Strangury Powder, and Wolly Grass Decoction for women. Infants get strangury by the toxicity of medicine.

  • Drain Kidneys Decoction 瀉腎湯 사신탕: All strangury involves distending contracture of the lower abdomen. As the symptom gets worse, use this medicine. Rhei Radix et Rhizoma (大黃) (chopped and soaked in a sealed box for a night] 2 don, Magnetitum (磁石) (broken) 1.6 don, Acori Gramineri Rhizoma (石菖蒲), Rehmanniae Radix Crudus (生地黃) 1 don each, Scrophulariae Radix (玄參), Asiasari Radix et Rhizoma (細辛) 8 pun each, Natrii Sulfas (芒硝), Poria Sclerotium Rubra (赤茯苓), Scutellariae Radix (黃芩) 6 pun each, and Glycyrrhizae Radix (甘草) 4 pun. Chop and steep these with 2 glasses of water until 1.5 remains. Add Rhei Radix et Rhizoma (大黃), steep until 7/10 remains, remove the leftovers, add Natrii Sulfas (芒硝), stir, and take on an empty stomach.
  • Turmeric and Coptis Pill 鬱金黃連丸 울금황련환: Used for accumulated heat in the small intestine or bladder. The following are all heat syndrome so use this medicine: difficult urination caused by uroschesis and ischuria, enuresis, incontinence of urine, turbid urine like un-boiled rice water, unctuous strangury like a pus, urine like water steeped with Gardeniae Fructus (梔子), urine with sand, a grain of rice, or starch. Talcum (滑石), Poria Sclerotium (白茯苓) 4 nyang each, Pharbitidis Semen (黑丑) (use the first-coming powder when ground and sifted) 3 nyang, Scutellariae Radix (黃芩), Rhei Radix et Rhizoma (大黃), Succinum (琥珀) 2 nyang each, Curcumae Radix (鬱金), and Coptidis Rhizoma (黃連) 1 nyang each. Grind into powder, make into paste with water, and make these into pills as big as a seed of the royal foxglove tree. Take 50 to 70 pills with boiled water.
  • Three Factors Amber Powder 三因琥珀散 삼인호박산: Used for difficult, painful urination with bloody pus. Succinum (琥珀), Lygodii Spora (海金沙), Myrrha (沒藥), and Typhae Pollen (蒲黃) all the same amount. Grind into powder, mix each 3 don with water steeped with Hemerocallidis Radix (萱草根), and take it on an empty stomach.
  • Pyrrosia Powder 石韋散 석위산: ① Used for all strangury. Talcum (滑石) 2 don, Atractylodis Rhizoma Alba (白朮), Dianthi Herba (瞿麥), Paeoniae Radix (赤芍藥), Malvae Semen (冬葵子), Pyrrosiae Folium (石韋), Akebiae Caulis (木通) 1 don each, Angelica Gigantis Radix (當歸), Melandrii Herba (王不留行), and Glycyrrhizae Radix (甘草) 5 pun each. Grind, mix each 2 don with water boiled with wheat, and take it on an empty stomach.

② Or slice and steep 1 nyang with water and take it.

  • Five Stranguries Powder 五淋散 오림산: Used for the five stranguries. Paeoniae Radix (赤芍藥), Gardeniae Fructus (梔子仁) 2 don each, Angelica Gigantis Radix (當歸), Poria Sclerotium Rubra (赤茯苓) 1 don each, Scutellariae Radix (黃芩) (thin and filled up), and Glycyrrhizae Radix (甘草) 5 pun each. Chop into 1 package, steep with water, and take it on an empty stomach.
  • Rice Paper-Plant Pith Decoction 通草湯 통초탕: Used for the five stranguries. Tetrapanacis Medulla (通草), Malvae Semen (冬葵子), Imperatae Rhizoma (茅根), Resina Amygdali (桃膠), Dianthi Herba (瞿麥), Angelica Gigantis Radix (當歸), Typhae Pollen (蒲黃), Talcum (滑石), Melandrii Herba (王不留行) 1 don each, and Glycyrrhizae Radix (甘草) 5 pun. Chop these into 1 package, steep with water with 5 pieces of Zingiberis Rhizoma (生薑), and take it on an empty stomach.
  • Definite Effect Powder 必效散 필효산: Used for all strangury. Angelica Gigantis Radix (當歸), Rehmanniae Radix Crudus (生地黃), Poria Sclerotium Rubra (赤茯苓), Talcum (滑石), Achyranthis Radix (牛膝), Gardeniae Fructus (梔子仁), Liriopis Tuber (麥門冬), Aruantii Fructus Immaturus (枳殼), Polygoni Avicularis Herba (萹蓄), Akebiae Caulis (木通), Anemarrhenae Rhizoma (知母), and Phellodendri Cortex (黃柏) [soaked in wine and roasted together] 7 pun each, and Glycyrrhizae Radix (甘草) 5 pun. Chop these into 1 package, add Junci Medulla (燈心) 1 dan, steep with water, and take it on an empty stomach.
  • Lygodium Fern Spores Powder 海金沙散 해금사산: Used for the five stranguries. Taking once can have a wonderful effect. Angelica Gigantis Radix (當歸) (washed in wine), Rhei Radix et Rhizoma (大黃) (soaked in wine), Achyranthis Radix (牛膝) (washed in wine), Aucklandiae Radix (木香), Realgar (雄黃), and Lygodii Spora (海金沙) 5 don each. Grind, mix each 2 don with good wine, and take it before going to bed.
  • Danliao’s Strangury Powder 澹寮五淋散 담료오림산: Used for the five stranguries. Gardeniae Fructus (梔子仁) 1.5 don, Poria Sclerotium Rubra (赤茯苓), Paeoniae Radix (赤芍藥) 1 don each, Akebiae Caulis (木通), Talcum (滑石), Glycyrrhizae Radix (甘草) 8 pun each and Phyllostachys Folium (竹葉) Artemisiae Capillaris Herba (茵蔯) 5 pun each. Chop into 1 package, steep with water, and take it on an empty stomach.
  • Another Prescription 一方 또 다른 처방: Used for all strangury. Four Ingredients with Poria Powder 4 don, Augment Base Powder 2 don and Gardeniae Fructus (梔子仁) 1 don. Chop into 1 package, steep with water, and take it on an empty stomach.
  • Wooly Grass Decoction 白茅湯 백모탕: ① Used for all strangury in women after delivery. All types including unctuous stone, cold, and heat strangury may be cured. Imperatae Rhizoma (白茅根) 5 don, Dianthi Herba (瞿麥), Poria Sclerotium (白茯苓) 2.5 don each, Malvae Semen (冬葵子), Ginseng Radix (人參) 1.25 don each, Typhae Pollen (蒲黃), Resina Amygdali (桃膠), Talcum (滑石) 7 pun each, Glycyrrhizae Radix (甘草) 5 pun, 2 Concha Cypraeae Violacae (紫貝) (heated), and 4 bones inside the head of a yellow croaker (heated). Chop these into 2 packages, add 3 pieces of Zingiberis Rhizoma (生薑) and 20 stems of Junci Medulla (燈心), steep with water and take it on an empty stomach.② Or grind into powder, mix with Junci Medulla (燈心)-steeped water and take it.

15-1 Strangury by Toxicity of Medicine in a Child 小兒藥毒成淋 소아가 약독으로 임병이 된 것

One child had suffered from strangury from the 7th month since birth, suffering from it once every 5-7 days. It was so painful that he groped the ground, screamed, and only got better after having urinated 1 glass of urine that looked like lacquer tree juice and millet. There was a rough pulse on pushing softly and a string-like pulse on pushing deep. He was slim, a little tall, and his face was pale and dark. His father must have had too much medicine for his lower energizer; heat must have stayed in the placenta and moved to the child’s life gate. Purple Snow (紫雪) and Phellodendri Cortex (黃柏) were ground into fine powder, mixed and made into pills as big as a seed of the royal foxglove tree, after which they were dried in the sun sufficiently. The child was given 200 pills and another 300 pills again in 4 hours. Then, the child was fed some hot water and food to press the medicine qi. After a half day, he got heavy pain within his waist and abdomen, urinating 1 bowl of urine that looked like lacquer tree juice and millet to alleviate the pain by 80%. Then, 1 package with Citrus Unshius Pericarpium (陳皮) 1 nyang, Platycodonis Radix (桔梗) and Akebiae Caulis (木通) a half nyang each was given to him. He was cured after urinating a cup of urine that looked like lacquer tree juice and a millet.
