A Manual For Sasang Constitutional Medicine ② Stages Show How A Symptom Could Be Developed On Soeum Type

Soeum type is the most delicate constitutional type and tends to be physically weaker and most vulnerable to stressors than other types.

This month, the clinical stage, characteristics of each stage, and formulas that treat each symptom in a stage of Soeum type will be introduced. All information was based on the evidence-based manual for diagnosis and treatment by acupuncture and herbal medicine.

The clinical stages are Congestive hyperpsychotic disease initial and final stages, Greater yang disease reverting yin pattern, Yang depletion disease intermediate stage and terminal stage, Exterior or interior disease unfavorable pattern, and Greater yin disease. And in the Greater yin disease, nine more sub-pattern types are based on the symptom. 

Followings are the clinical stage of Soeum type. Characteristics and herbal formulas that should be used are explained.

 

▲ Congestive hyperpsychotic disease

▷Congestive hyperpsychotic disease initial stage pattern: a pattern characterized by Exterior heat symptoms such as body heat including fever, chilling, no sweat, and body aches. Cheongunggyeji-tang and Gungguihyangso-san should be considered for the treatment.

▷Congestive hyperpsychotic disease intermediate stage pattern: Fever, no chilling, no sweat, abdominal distension, constipation, irritability, and restlessness are characteristics. Palmulgunja-tang and Hyangbujapalmul-tang should be considered. Hyangbujapalmul-tang may be applied to insomnia, upward rush, headache, irritably heat sensation of hands and feet, anxiety due to excessive stress, etc.

▷Congestive hyperpsychotic disease terminal stage pattern: The pattern in which Exterior heat symptoms similar to initial stage pattern but has become extremely severe. Doksampalmulgunja-tang should be considered for the treatment of the stage pattern.

▷Greater yang disease reverting yin pattern: a pattern includes deadly cold hands and feet, lower abdominal pain, lower abdominal stuffiness, curled-up tongue, and contraction of the scrotum that appeared suddenly. Doksampalmulgunja-tang and Insamosuyu-tang should be used.

▷Yang depletion disease initial stage pattern: a pattern characterized by body heat, including fever, chilling, body aches, and spontaneous sweating. Hwanggigyeji-tang should be prescirbed.

▷Yang depletion disease intermediate stage pattern: a pattern characterized by body heat.  Bojungikgi-tang and Seungyangikgi-tang should be considered.

▷Yang depletion disease terminal stage pattern: a pattern characterized by body heat including fever, abdominal distension, constipation, extreme irritability, restlessness, increased spontaneous sweating and decreased urine output, and scantly dark urine. Hwanggigeyjibuja-tang and Seungyangikgibuja-tang are used.

 

▲ Greater yin disease

▷Greater yin disease Diarrhea pattern: a pattern that mainly focuses on abdominal distension, abdominal pain, and diarrhea Baekhaoijung-tang and Baekhaobujaijung-tang should be considered for the treatment.

▷Greater yin disease Epigastric stuffiness and fullness pattern: the stuffy symptom as if the chest or the epigastric part is clogged is added. Gwakhyangjeonggi-san should be considered for selection.

Greater yin disease Jaundice pattern is characterized by adding jaundice or skin color change. Injingyulpi-tang is considered.

Greater yin disease Edema pattern: adding abdominal distension and edema in Soeum type Greater yin disease. Sibimigwanjung-tang, Gyebugwakjinijung-tang and Gungguichongsoijung-tang is considered.

Greater yin disease Yin toxin pattern: the sudden appearance of a dark blue face, lips, and nails, pain as if the body has been beaten, coldness of the limbs, and a desire to continue sleeping. Insamjinpi-tang and Insamgyepi-tang should be considered.

 

▲ Lesser yin disease

Lesser yin disease Diarrhea pattern: abdominal pain, diarrhea accompanied by dry mouth, mouth stuffy and uncomfortable. Gwangyebujaijung-tang should be considered. Lesser yin disease Blue tinged watery diarrhea pattern: a pattern with severe bluish watery diarrhea. Gwangyebujaijung-tang should be used.

Lesser yin disease Visceral Qi collapse and reversal pattern: a pattern accompanied by symptoms of being unable to sit still due to prolonged mania and Gwol syndrome. Gwangyebujaijung-tang should be used.

Lesser yin disease Exuberant yin and repelled yang pattern: a symptom has reached a very serious condition. Osuyubujaijung-tang should be used.
