21 Headaches of the Six Meridians Should Be Discriminated 頭痛當分六經 두통은 육경을 구분해야 한다

① When they say all headaches can be cured with Wind-treating drugs, it is because the head is located up high where only the Wind can reach it. However, there is a difference between the three yang and three yin.

② Greater yang headaches are distinguished by an aversion to Wind-Cold and a floating and forceful (浮緊) pulse; use Osterici Radix (羌活), Araliae Continentalis Radix (獨活), Ephedrae Herba (麻黃), Cnidii Rhizoma (川芎) and such.

③ Lesser yang headaches are distinguished by feeling cold and heat repeatedly and a string-like, fine (弦細) pulse; use Bupleuri Radix (柴胡), Scutellariae Radix (黃芩) and such.

④ yang brightness headaches are distinguished by spontaneous sweating, fever, and a moderate, long and replete (緩長實) pulse; use Puerariae Radix (葛根), Cimicifugae Rhizoma (升麻), Gypsum Fibrosum (石膏), Angelicae Dahuricae Radix (白芷) and such.

⑤ Greater yin headaches are distinguished by phlegm, heaviness in the body, and a sunken and moderate pulse; use Atractylodis Rhizoma (蒼朮), Pinelliae Tuber (半夏), Arisaematis Rhizoma (南星) and such.

⑥ Lesser yin headaches are distinguished by icy feet due to obstructions in the three yin meridians and three yang meridians, cold reversal caused by qi counterflow and a sunken, fine pulse; use an Ephedra, Aconite and Asarum Decoction. 

⑦ Reverting yin headaches are distinguished by the vomiting of phlegm or foam, reversal cold of the extremities, and a floating and moderate pulse. Use an Evodia Decoction.

⑧ When there is a headache due to a three yang combination disease, use a Triple Yang Decoction.

⑨ When the patient cannot bear to look at sunlight and needs ice on the head due to a headache caused by heat accumulation in the three yangs, use a combination of promoting sweating, emetic, and purgation therapies in treatment.

  • Triple Yang Decoction 三陽湯 삼양탕: It treats headaches due to a three yang combination disease. Osterici Radix (羌活), Saposhnikoviae Radix (防風), Gypsum Fibrosum (石膏), Angelicae Dahuricae Radix (白芷), Bupleuri Radix (柴胡), Cnidii Rhizoma (川芎) 1 don each, and Schizonepetae Spica (荊芥), Cimicifugae Rhizoma (升麻), Puerariae Radix (葛根), Paeoniae Radix (芍藥), Asiasari Radix et Rhizoma (細辛) 5 pun each. Slice these drugs to make 1 package, add 3 Allii Fistulosi Bulbus (葱白) with the stem still intact, and decoct in water.

22 Brain Wind Syndrome 腦風證 뇌풍증

① When Wind qi travels up to GV16, it becomes brain wind.

② The symptoms include chills in the nape, spine and GV17 area. Use Divine Sages Powder, Greater Yang Elixir. 『Liu Hejian』

  • Divine Sages Powder 神聖散 신성산: It treats brain wind (腦風). Equal amounts of Ephedrae Herba (麻黃), Asiasari Radix et Rhizoma (細辛), Puerariae Radix (葛根) (half raw and half roasted), Agastachis Herba (藿香). Grind these drugs and add to alcohol made with 2 don each of Schizonepetae Spica (荊芥) and Menthae Herba (薄荷). 『Liu Hejian』
  • Greater Yang Elixir 太陽丹 태양단: Brain Cold disease (腦寒病) show symptoms of discomfort night and day due to headaches caused by the upper energizer attacking the brain. This is the main prescription. Gypsum Fibrosum (石膏) 2 nyang, and Cnidii Rhizoma (川芎), Aconiti Tuber (川烏) (baked wrapped in wet paper), Angelicae Dahuricae Radix (白芷), Glycyrrhizae Radix (甘草) 1 nyang each, Borneolum (龍腦) 2 don. Grind these drugs, knead in boiled down honey and flour paste, make into 18 pills per 1 nyang, and cover with Minium (黃丹). Take 2~3 pills at a time after meals with water decocted with welsh onions and tea.

23 The Syndrome Caused by the Wind Pathogenic Factor Attacking the Head 首風證 수풍증

① Exposure to Wind directly after bathing causes a syndrome caused by the Wind pathogenic factor attacking the head. The symptoms include heavy perspiration on the head and face, an aversion to wind, and worsening of the headache the day before a windy day, which does not permit the person to come and go. However, on the day of the wind, the symptoms are somewhat alleviated.

② A Great Chuanxiong Pill is mainly used. 『Liu Hejian』

③ For dizziness and a headache occurring after taking a bath, use an Angelica Root Pill.

  • Great Chuanxiong Pill 大川芎丸 대천궁환: It treats the syndrome caused by the Wind pathogenic factor attacking the head. Grind 1 nyang each of Cnidii Rhizoma (川芎) 4 nyang and Gastrodiae Rhizoma (天麻), knead in honey, make into 10 pills per 1 nyang, and take with tea or alcohol, 1 pill at a time.『Liu Hejian』
  • Angelica Root Pill 白芷丸 백지환: ① When there is dizziness and headaches after bathing due to head wind, take this to brighten the eyes. It is also effective for cases where the patient feels suddenly cold but then feels temporarily warm, experiences confusion and faintness and dizziness in the head and eyes. Prepare an appropriate amount of fresh Angelicae Dahuricae Radix (白芷), slice, soak in welsh white radish juice, dry in the sun, grind, and knead in honey to make pills the size of a bullet. Take 1 pill at a time with tea or water decocted with Schizonepetae Spica (荊芥). ② This is also called a Liang Capital Formula.

24 Supraorbital Pain 眉稜骨痛 미릉골통

① Pain in the region between the eyebrows and the eyebrow bone is due to the presence of phlegm. Take Qingzhou Typhonium Balls with water decocted with a Syzygii Flos Relieve Stomach Decoction.

② For severe pain in the eyebrow bone that travels down to the eye causing inability to open them, which calms down during the day but worsens during the night, or pain in the eyebrow and bones around the eye socket and heaviness in the body due to damp phlegm, add Aconiti Tuber (川烏), Atractylodis Rhizoma Alba (白朮) to a Chuanxiong and Magnolia Flower Guide Out Phlegm Decoction.

③ For supraorbital pain, use a Amazing Choice Decoction, Clear Upper Powder.

④ Pain in the eyebrow and the eyeball areas belong to wind heat and phlegm. Grind equal amounts of Angelicae Dahuricae Radix (白芷), Scutellariae Radix (黃芩) (thick with soft inside; processed with alcohol) and add to tea 2 don at a time to drink. It is also effective with alcohol.

⑤ Pain in the bones around the eyebrow is caused by phlegm Fire. Pain in the eyebrow as well as eyeballs is also a symptom of phlegm Fire.

⑥ For pain in the supraorbital bone due to wind cold, add 1 don each of Aconiti Tuber (川烏) and Aconiti Ciliare Tuber (草烏) (all soaked in urine of a young boy for 2 nights and roasted) to Asiasari Radix et Rhizoma (細辛), Osterici Radix (羌活), Scutellariae Radix (黃芩) (thick with soft insides; processed with alcohol), Glycyrrhizae Radix (甘草) (baked) 0.5 don each and grind altogether, divide into two packages, and drink with tea water after meals.

  • Amazing Choice Decoction 選奇湯 선기탕: It treats unbearable pain in the supraorbital area. Osterici Radix (羌活), Saposhnikoviae Radix (防風), Pinelliae Tuber (半夏) (processed) 2 don each, Scutellariae Radix (黃芩) (thick with soft insides; processed with alcohol) 1.5 don, Glycyrrhizae Radix (甘草) 1 don. Slice these drugs to make 1 package, add 3 slices of Zingiberis Rhizoma Crudus (生薑) and decoct in water.
  • Clear Upper Powder 上淸散 상청산: It treats unbearable pain in the eyebrow bone and eyeball areas due to a Wind headache. Cnidii Rhizoma (川芎), Curcumae Radix (鬱金), Paeoniae Radix (芍藥), Schizonepetae Spica (荊芥), Menthae Herba (薄荷), Natrii Sulfas (芒硝) 2.5 don each, Olibanum (乳香), Myrrha (沒藥) 5 pun each, and Borneolum (龍腦) 2.5 pun. Grind these drugs and insert into the nostrils 2.5 pun at a time to induce sneezing.

25 Incurable and Obstinate Diseases of the Head 頭痛不治難治證 두통의 불치증과 난치증

① The true headache is a severe headache where the whole brain hurts. The hands and feet turn cold, and if the chills travel down to the joints, it is incurable and the patient dies.

② In the case of a true headache, the pain penetrates the GV26 and enters the Ni Hwan Palace (泥丸宮); it cannot be treated with medicine. Therefore, when this occurs in the morning, the patient dies in the evening. When this occurs in the evening, the patient dies the following morning. This is because the head is the base foundation of a person, and qi in this foundation has been broken off.

③ If the head and brain are severely painful and the hands and feet all feel icy, it cannot be treated.

④ Deep pain in the head that invades Ni Hwan Palace (泥丸宮) to cause icy hands and feet and bluish nails is called a ‘true headache’. The pain travels down to the teeth and causes severe pain. This belongs to the lesser yin reversal syndrome. All of this cannot be treated.

⑤ If the pain in the head and eyes is chronic and one cannot see anything, it indicates death is near.

⑥ When the head and eyes hurt so severly that nothing can be seen, the patient will die.

⑦ If there is dizziness accompanied by massive diarrhea and occasional faintness in the eyes, it is difficult to treat.

⑧ In the case of dizziness accompanied by linguistic difficulties, massive perspiration, diarrhea and occasional faintness in the eyes, it can be diagnosed as a severe deficiency syndrome. It is difficult to cure.

26 Tremors of the Head 風頭旋 머리를 흔드는 것

① Tremors of the head do not have any other distinctive symptoms except constantly shaking the head.

② If the liver-Wind is exuberant, one shakes the head.

③ The treatment methods are identical to those of head wind.

④ One patient constantly shook his head for 7 years and had hematochezia for 3 years. Many different treatment methods turned out to be ineffective, leaving me to think that it is caused by wind heat from the exterior invading exuberant liver blood. The liver belongs to Wood, and when Wood is exuberant, the spleen Earth is suppressed. Both the spleen and the lungs, son and mother, are suppressed by the liver and thus the blood seeps into the feces, causing continuous hematochezia. A prescription that removes liver-Wind and replenishes the spleen at the same time cured the symptoms, with only a few packages. 10 days later, the hematochezia stopped, followed by diarrhea with white pus, after which the patient became well.

⑤ Grind Saposhnikoviae Radix (防風) 3 nyang, Trichosanthis Radix (天花粉), Astragali Radix (黃芪) (soaked in honey and roasted), Osterici Radix (羌活), Paeoniae Radix (芍藥) 5 don each, Rhinocertis Cornu (犀角) (carved up and ground), Glycyrrhizae Radix (甘草) 2.5 don each, and Serpentis Periostracum (蛇退) (baked), red Uncariae Ramulus et Uncus (釣藤), Ephedrae Herba (麻黃) 1 don each and knead in Zizyphi Fructus (大棗) to make pills the size of the seed of the royal foxglove tree. Take with a Menthae Herba decoction 50~70 pills at a time after meals. The shaking of the head stops after just 2 doses and hematochezia is treated as well.

27 Opening of the Fonatelle 腦縫開裂 뇌봉이 열린 것

When the fonatelle opens up due to severe heat, mash and apply 300~500 lice.

28 Dandruff in the Hair 頭生白屑머리에 비듬이 생기는 것

① Dandruff is a symptom of the lungs. Because the lungs govern the skin and hair, if there is wind heat, the scalp becomes dry and itchy forming dandruff. Eliminate Wind Powder is mainly used.

② When there is dandruff and the scalp is extremely dry and itchy due to a head wind, ground Veratri Rhizoma et Radix (藜蘆) and use it. First, wash the hair and wait until it dries. Then, apply the powder so that it is properly applied onto the scalp. Wrap the head for 2 nights and the dryness and the itchiness will be cured. When it is ineffective, repeat the steps and first wash the hair in water brewed with Veratri Rhizoma et Radix (藜蘆) before applying, for miraculous effects.

③ Also, powder Angelicae Dahuricae Radix (白芷), Lysimachiae Foenum-Graeci Herba (零陵香) and sprinkle on the head, and comb the hair 3~5 days later with a very dense comb to remove the lice. Repeat 2~3 times to see effects. ④ When there is dandruff due to head wind, wash the hair 6~7 times with lye made from Orostachys Herba (瓦松) dried out in the sun. ⑤ Rub the hair with oil from a bear’s brain to remove dandruff.
