19 Itching and Pain Inside the Penis 莖中痒痛 음경 속이 가렵고 아픈 것
① When a child has sex before reaching adulthood or a senile man thinks of women and sends down sperm despite having a shrunken penis, the essence does not come out and becomes rotten, leading to pain inside the penis, difficulty in urination, and strangury diseases. Steep the Eight Ingredients Pill (八味丸) material with Plantaginis Semen (車前子) and Achyranthis Radix (牛膝) and take it.
② If already exhausted essence gets used again, one may have contracture and pain in the routes that feces and urine come out through. When more pain begets more urgency to defecate or urinate, and the more excretion begets more pain, use an Eight Ingredients Pill (八味丸) [the prescription is seen in the chapter on Five Viscera] with Aconiti Lateralis Radix Preparata (附子) in twofold. In this case, the origin of change must be nurtured. Never use Anemarrhenae Rhizoma (知母), Phellodendri Cortex (黃柏), or water-draining medicines that have a bland taste. It’s because genuine yang has already gone out and doing that could damage genuine yin.
③ Use a Ginseng and Amber Decoction for contracture and stabbing pain reaching from the penis to the hypochondrium caused by blockage and heat in the liver meridian.
④ Steep a Six Gentlemen Decoction [the prescription is seen in the chapter on Phlegm] with Anemarrhenae Rhizoma (知母), Phellodendri Cortex (黃柏), Talcum (滑石), Pyrrosiae Folium (石韋), Succinum (琥珀), and take it for unbearable pain inside the penis caused by a strangury disease.
⑤ Steep a Minor Bupleurum Decoction [the prescription is seen in the chapter on Cold Damage] with Gardeniae Fructus (梔子), Alismatis Rhizoma (澤瀉), Coptidis Rhizoma (黃連) (baked), Akebiae Caulis (木通), Gentianae Radix (草龍膽), Poria Sclerotium Rubra (赤茯苓) for painful urination, white ooze, blockage of urination, and occasional itching. Take it with a Six Ingredients Rehmannia Pill (六味地黃丸). [The prescription is seen in the chapter on Five Viscera]
⑥ Because dribbling of urination and pain inside the penis belong to dampness in the liver meridian, use a Gentian Drain Liver Decoction, [The prescription is seen in the chapter on Genitals].
⑦ Use an Eight Ingredients Pill for exhausted essence and painless itching felt inside the penis. Use a Guide Out Red Formula [the prescription is seen in the chapter on Five Viscera] with Gardeniae Fructus (梔子) and Rhei Radix et Rhizoma (大黃) used for prosperous heat and roughness and pain in the penis.
⑧ An itching penis with white ooze is mostly due to the deficiency of the spleen Earth not being able to feed Metal and Water. It’s also caused when a blood deficiency of the liver meridian leads to Fire dryness. Use a Tonify the Center to Augment the qi Decoction and Clear Heart Lotus Seed Drink in turn. The origins of the liver and kidneys are the spleen and stomach but actually the heart governs these.
⑨ Pain inside the penis is due to blockage of the liver meridian and heat. Using Glycyrrhizae Radix (甘草) (small roots) in this case is to ease the qi.
Ginseng and Amber Decoction (參苓琥珀湯 삼령호박탕): Used for the dribbling of urination and unbearable pain inside the penis.
- Meliae Fructus (川練肉), Glycyrrhizae Radix (甘草) (small roots) 1 don each
- Corydalis Tuber (玄胡索) 7 pun
- Ginseng Radix (人參) 5 pun
- Poria Sclerotium Rubra (赤茯苓) 4 pun
Succinum (琥珀), Alismatis Rhizoma (澤瀉), Bupleuri Radix (柴胡), Angelica Gigantis Radix (當歸) (small roots), Citri Unshius Pericarpium Immaturus (靑皮), Phellodendri Cortex (黃柏) 3 pun each Chop into 1 package, add 1 dan of Junci Medulla (燈心), steep with water, and take on an empty stomach.
A Prescription (一方 또 다른 처방): Used for the dribbling of urination and pain inside the penis.
- Glycyrrhizae Radix (甘草) (small roots) and Akebiae Caulis (木通) 2 don
- Citri Unshius Pericarpium Immaturus (靑皮), Phellodendri Cortex (黃柏), Alismatis Rhizoma (澤瀉) 1 don each Chop into 1 package, steep with water, and take it on an empty stomach.
20 Recto-urethral Fistula, Communicating Bowels Syndrome 交腸證 교장증
① In women, the symptom of feces appearing in urination is called recto-urethral fistula. Taking Five Ingredients Powder with Poria is efficacious. If not, mix ashes from burning an old hood with wine and take it.
② After being cured, one woman excreted feces with urination. This is because yin and yang lost the ability to deliver and transfer. It is called ‘Daesojangkyo (大小腸交, Communicating bowls).’ After taking Five Ingredients Powder with Poria [the prescription is seen in the chapter on Cold Damage] 2 je and then a Tonify the Center to Augment the qi Decoction, she recovered.
③ One woman was fond of wine and never got drunk even after drinking a lot. One day, feces came out through the anterior yin and urine came out through the posterior yin. All six pulses were sunken and rough. Then, after steeping a Four Substances Decoction with Lygodii Spora (海金沙), Aucklandiae Radix (木香), Arecae Semen (檳榔), Akebiae Caulis (木通), Persicae Semen (桃仁) and taking it, she recovered.
This woman drank too much which caused qi to just ascend and not descend. This made yang extremely deficient. In addition, yin also became extremely deficient as the heat from the stagnation of dampness due to immoderate drinking boiled the blood down. Though both yin and yang became deficient, she could survive for some time thanks to her excess body constituent and grain qi in wine. She was told that she would die in 3 months, and so it went.
21 The Amount of Urine 小便多寡 소변의 양
Why do the old urinate more and the young urinate less even when they drink the same amounts of water?
Qi of the young is the same as that of spring and summer: there is much ascending and little descending.
Qi of the old is the same as that of autumn and winter: there is much descending and little ascending.
Therefore, the amount of urine is not the same.
22 Urinating as Soon as Drinking Water 飮後卽小便 물을 마시면 소변이 바로 나오는 것
① The Inner Classic says, “When water enters the stomach and essential qi overflows up to the spleen, the spleen qi scatters and lifts it up to the lungs.”
When a patient drinks water, feels it entering the stomach, and reaching up to lower part of the umbilicus, and immediately needs to urinate, it is because essential qi cannot enter the spleen and the lungs. Therefore, the heart Fire attacks upward to make the mouth and throat dry. Use a Tonify the Center to Augment the qi Decoction.
② The Divine Pivot says, “When people drink wine, it goes into the stomach too. Why does urine come first before grains are digested? Wine is liquid fermented from grains; its qi is swift and clear. Therefore, even if it enters later than grains, it comes out through urine before grains.”
