12 Various Strangury Symptoms 諸淋證 여러 가지 임증
① Strangury diseases is when a millet-like object is found in urine and contracture and pain are felt from the lower abdomen to the umbilicus.
② All is due to a kidney deficiency and heat in the bladder. As heart and kidney qi are blocked and accumulated in the lower energizer, the bladder contracts, and urine with oil, blood or stones come out. Difficulty in urination or dribbling of urine occurs. In the more severe cases, blockage causes pain, and tightness leads to fainting.
③ When qi is in the small intestine, urine leads to a swollen abdomen. When blood is in the small intestine, there is difficulty in urination. When heat is in the small intestine, urination is painful. Painful urination is blood strangury, while painfree urination is hematuria. One formed by essence damage is sandy strangury; one formed and spread by essence is unctuous strangury; one formed by iron or stone is stone strangury.
④ In this case, never use tonifying medicine. Tonifying qi makes the abdomen get more swollen, tonifying blood causes more obstruction, and tonifying heat makes it more severe so that not only urine but also defecation may get blocked too. I have never seen such a case to survive.
⑤ Strangury disease involves dribbling, painful, and difficult urination. There is also an urge to urinate even after having just urinated.
⑥ All strangury diseases belong to heat but occasionally cold may be the cause. It is due to the ineffective operation of the small intestine and bladder, caused by the blockages of heart and kidney qi. There could be other causes: dampness-heat flowing downward and invading the liver meridian due to indulgences in one’s sexual life, holding urine, eating wine and meat, or anger. This will eventually block the genitals, which will initially cause heat and blood strangury; Fire will boil them and make them into sandy and stone strangury. It is just like stones appearing when medicine is boiled for a long time in a medical herb extractor.
⑦ Tonify kidney essence and ease urination in the case of strangury disease caused by a kidney deficiency. Never use only diuretics.
⑧ The five strangury diseases come from heat accumulation in the bladder.
⑨ When distention and fullness of the lower abdomen is severe, use a Drain Kidneys Decoction.
⑩ All strangury belongs to heat. Although there is cold strangury, only one in a hunded or thousand cases are caused by cold.
⑪ Despite there being five strangury types, all belong to heat. Prescribe a Nourish Kidneys Pill (100 pills) with water steeped with a Four Substances Decoction in addition to Glycyrrhizae Radix (甘草) (small roots), Polygoni Cuspidati Radix (虎杖根), Akebiae Caulis (木通), Persicae Semen (桃仁), Talcum (滑石), Aucklandiae Radix (木香), and burn moxa on SP6 [name of an acupuncture point]. I tried it many times and it was efficacious.
⑫ Never induce sweating for a strangury disease. Forcing sweating results in bloody stools.
13 Five Strangury Diseases 淋病有五 5가지 임병
First is overstrain strangury, second is blood strangury, third is heat strangury, fourth is qi strangury, and fifth is stone strangury.
13-1 Overstrain Strangury 勞淋 노림
① Overstrain strangury involves dysuria, contracture, and pain in the lower abdomen.
② It occurs as a result of indulgences in one’s sexual life. There is pain and contracture on ST30[name of an acupuncture point], reaching up to the coccyx.
③ Use Four Substances Decoction (四物湯) [the prescription is mentioned in the chapter on Blood] with Anemarrhenae Rhizoma (知母), Phellodendri Cortex (黃柏), Talcum (滑石), and Succinum (琥珀) for overstrian strangury from fatigue due to overexertion.
④ In the case of a strangury disease caused by excessive wine or sexual desire damaging and obstructing bladder qi, use a Tonify the Center to Augment the qi Decoction (補中益氣湯) [the prescription is mentioned in the chapter on Internal Damage] to tonify and lift genuine qi up.
⑤ If ejaculation is held by force during sexual intercourse, it overflows downward to permeate and becomes a strangury disease. Use an Augment the Source and Stabilize the True Decoction.
- Augment Source and Stabilize Origin Decoction 益元固眞湯 익원고진탕: If ejaculation is held by force during sexual intercourse, it overflows downward to permeate and becomes a strangury disease. This medicine cures it. Glycyrrhizae Radix (甘草) (small roots) 2 don, Dioscoreae Rhizoma (山藥), Alismatis Rhizoma (澤瀉) 1.5 don each, Ginseng Radix (人參), Poria Sclerotium (白茯苓), Nelumbinis Stamen (蓮花蘂), Morindae Radix (巴戟), Cimicifugae Rhizoma (升麻), Alpiniae Oxyphyllae Fructus (益智仁), and Phellodendri Cortex (黃柏) (soaked in wine and baked) 1 don each. Chop into 1 package, steep with water, and take it on an empty stomach.
13-2 Blood Strangury 血淋 혈림
① Blood strangury involves difficulty in urination, occasional hematochezia, pain, fullness, and contracture.
② It is caused by heat. In severe cases, hematuria occurs and the penis is painful because of heat stagnation. You must use a Four Substances Decoction (四物湯) with Anemarrhenae Rhizoma (知母), Phellodendri Cortex (黃柏), Alismatis Rhizoma (澤瀉), and Poria Sclerotium Rubra (赤茯苓).
③ The bright reddish blood means there is deficiency heat in the heart and small intestine, so subtract Glycyrrhizae Radix (甘草) from a Guide Out Red Formula [the prescription is seen in the chapter on Five Viscera] and add Scutellariae Radix (黃芩). The blood being like black bean juice means there is Fire in the kidneys and bladder, so use Five Stranguries Powder [the prescription appears later].
④ Use Guide Out Red Powder to Add Flavor, Golden Rehmannia Decoction, and Cephalonoplosi Herba Pill for blood strangury.
⑤ Achyranthes Paste (牛膝膏) [the prescription appears later] is the most efficacious for strangury caused by static blood. [The prescription is seen below].
- Supplemented Guide Out Red Powder 增味導赤散 증미도적산: Used for difficult and painful urination caused by blood strangury. Rehmanniae Radix (生乾地黃), Akebiae Caulis (木通), Scutellariae Radix (黃芩), Plantaginis Semen (車前子), Gardeniae Fructus (梔子) (梔子仁), Cnidii Rhizoma (川芎), Paeoniae Radix (赤芍藥) and Glycyrrhizae Radix (甘草) 1 don each. Chop into 1 package, steep with water with 3 pieces of Zingiberis Rhizoma (生薑) and 10 sheets of Phyllostachys Folium (竹葉), and take it on an empty stomach.
- Golden Rehmannia Decoction 金黃湯 금황탕: Used for bloody, difficult and painful urination. Curcumae Radix (鬱金), Dianthi Herba (瞿麥), Rehmanniae Radix (生乾地黃), Plantaginis Semen (車前子), Talcum (滑石), and Natrii Sulfas (芒硝) 5 don each. Grind these into powder, steep 5 don each with water, and take it on an empty stomach.
- Cephalonoplosi Herba Pill 小薊飮子 소계음자: Used for bloody, dribbling and painful urination caused by the accumulation of heat in the lower energizer. Rehmanniae Radix Crudus (生地黃) 2 don, Cephalonoplosi Radix (小薊根), Talcum (滑石), Tetrapanacis Medulla (通草), Typhae Pollen (蒲黃) (roasted), Nelumbinis Rhizomatis Nodus (藕節), Phyllostachys Folium (竹葉), Angelica Gigantis Radix (當歸), Gardeniae Fructus (梔子仁), and Glycyrrhizae Radix (甘草) (baked) 7 pun each. Chop into 1 package, steep with water, and take it on an empty stomach.
13-3 Heat Strangury 熱淋 열림
① Heat strangury involves reddish urine caused by heat, unpleasant dribbling of urination, contracture, and pain of the abdomen under the umbilicus.
② For sudden strangury with severe pain, reddish, and dribbling urination, use Eight Ingredient Rectify Powder [the prescription is mentioned previously] and Guide Out Red Formula [the prescription is seen later].
③ Steep Nourish Kidneys Pill [the prescription is seen later] 100 pills with water with a Guide Out Red Formula and Gardeniae Fructus (梔子) and take it on an empty stomach.
④ Grind Augment Base Powder [the prescription is seen in the chapter on Summer-heat] 2 don with Aucklandiae Radix (木香), Arecae Semen (檳榔) and Foeniculi Fructus (茴香) 1 don each, mix with boiled water, and take it on an empty stomach.
13-4 Qi Strangury 氣淋 기림
① For difficult and dribbling urination, distention, and fullness of the lower abdomen, use Aquilaria Powder and Unblock Secret Powder or grind Aucklandiae Radix (木香), Arecae Semen (檳榔), and Foeniculi Fructus (茴香) with Augment Base Powder (益元散) and take it in the same way as mentioned below.
② For a strangury disease caused by qi deficiency, steep an Eight Substances Decoction [the prescription is seen in the chapter on Deficiency Vexation] with Astragali Radix (黃芪), Polygoni Cuspidati Radix (虎杖根), Scutellariae Radix (黃芩), Achyranthis Radix (牛膝), and take it.
- Aquilaria Powder 沈香散 침향산: ① Used for distention and fullness of the lower abdomen by qi strangury. Malvae Semen (冬葵子) and Paeoniae Radix (赤芍藥) 7.5 don each. Aquillariae Lignum (沈香), Pyrrosiae Folium (石韋), Talcum (滑石), Melandrii Herba (王不留行), Angelica Gigantis Radix (當歸) 5 don each, and Citrus Unshius Pericarpium (陳皮), Citri Unshius Pericarpium Immaturus (靑皮), Aucklandiae Radix (木香), and Glycyrrhizae Radix (甘草) 2.5 don each. Grind into powder, steep with barley-boiled water, and take it on an empty stomach. ② Otherwise, chop, steep 1 nyang, and take it. This is called Chimhyangyum (沈香飮).
- Unblock Secret Powder 通秘散 통비산: Used for unbearable pain caused by qi strangury. Cyperi Rhizoma (香附子), Citrus Unshius Pericarpium (陳皮), and Poria Sclerotium Rubra (赤茯苓) all the same amounts. Chop, steep 5 don with water, and take it on an empty stomach.
13-5 Stone Strangury 石淋 석림
① Stone strangury involves pain inside the penis, difficulty during urination, contracture, and pain from the inside to the lower abdomen, sand or stones appearing in the urine, and an oppressed feeling leading to fainting.
② There is pain inside the penis and when urination is forced, sand or stones appear in the urine.
③ Sandy strangury is clotted oil that can be scattered, while stone strangury’s clotted mass cannot be scattered. For stone strangury, taking Recovery Elixir [the prescription is seen in the chapter on Qi] with Bitter Orange Powder [or Six to One Bitter Orange Powder] in boiled water is curiously efficacious. By taking this for a long time, all joints, orifices, meridians, and collaterals are opened to ease defecation and urine comes out with the stones.
④ Augment Base Powder is mainly used for stone strangury.
⑤ Use a Spirifer Fossil Pill, Talcum Powder, and Borax Powder for stone strangury.
- Spirifer Fossil Pill 石燕丸 석연환: Used for stone strangury. Fossilia Spirferis (石燕子) (Burnt reddish in the fire, soaked in vinegar three times, ground, refined with water, and then dried in the sun.), Talcum (滑石), Pyrrosiae Folium (石韋), and Dianthi Herba (瞿麥) 1 nyang each. Grind, make into paste with starch, and make these into pills as big as a seed of the royal foxglove tree. Take 30~50 pills twice on an empty stomach with water steeped with Dianthi Herba (瞿麥) and Junci Medulla (燈心).
- Talcum Powder 滑石散 활석산: Used for tearing pain caused by sandy and stony strangury. Talcum (滑石), Gypsum Fibrosum (石膏) 5 don each, Pyrrosiae Folium (石韋), Dianthi Herba (瞿麥), Akebiae Caulis (木通), and Althaeae Semen (蜀葵子) 3 don each. Grind, mix each 2 don with water boiled with 2 stems of Allii Fistulosi Bulbus (葱白), 1 handful of Junci Medulla (燈心) and 2 spoons of honey, and then take it on an empty stomach.
- Borax Powder 鵬砂散 붕사산: Used for contracture and pain caused by sandy and stone strangury. Borax (鵬砂), Succinum (琥珀), Poria Sclerotium Rubra (赤茯苓), Althaeae Semen (蜀葵子), and Citrus Unshius Pericarpium (陳皮) 3 don each. Grind these into powder, mix each 2 don with water boiled with 2 stems of Allii Fistulosi Bulbus (葱白), 21 pills of Liriopis Tuber (麥門冬) and 2 spoons of honey, and then take it on an empty stomach.
